We discuss the process underlying the success of an HIV-prevention project for young, inner-city women. The intervention was based on the concepts of empowerment and culturally sensitive skill building. Four critical points relevant to the translation of HIV prevention knowledge into behavioral change among the sample are examined: (1) integrating the important issues of the participants' lives into the HIV prevention program, (2) utilizing a group format to encourage cohesiveness and support, (3) engaging group facilitators to promote mutuality and equality, and (4) promoting ongoing, authentic relationships among the participants and staff members. Points are illustrated with vignettes reconstructed from the group facilitators' experiences with the participants.

Cytochrome b5 is a liver integral membrane protein that has now been expressed in, and isolated from, Escherichia coli. The structure-function relationships of the 43 amino acid membrane-binding domain (nonpolar peptide) have been examined in both native and mutant forms of the protein; in the latter, tryptophan residues at positions 108 and 112 were replaced by leucine. The temperature dependence of the fluorescence quantum yield of the Trp residues in the isolated membrane-binding domain was examined while the domain was bound to lipid vesicles. Both the lipid-bound mutant domain and lipid-bound native domain showed an irreversible increase in fluorescence above 50-degrees-C. When the whole cytochrome b5 molecule, bound to lipid vesicles, was heated to this temperature, there was a conversion of the metastable, intermembrane-exchangeable (''loosely'' bound), conformation to a final, virtually unexchangeable (''tightly'' bound), conformation. It has been suggested previously that the protein exists in a ''looped back'' conformation and a ''bilayer penetrating'' conformation. Although the present studies are not designed to determine the absolute conformations of the loose and tight forms, the changes observed in steady-state and frequency-modulated fluorescence and the lack of change in depth of Trp 109 in the bilayer are consistent with a movement of the C-terminal segment from a looped back to a bilayer penetrating conformation as the tight form is generated.

The response of developing muscle spindles to denervation was studied by sectioning the nerve to the medial gastrocnemius muscle of rats at birth. The denervated spindles were examined daily throughout the first postnatal week for changes in ultrastructure and expression of several isoforms of myosin heavy chain (MHC). Each of the three different types of intrafusal muscle fiber exhibited a different response to denervation. Within 5 days after the nerve section nuclear bag, fibers degenerated completely; nuclear bag1 fibers persisted, but ceased to express the 'spindle-specific' slow-tonic MHC isoform and thereby could not be differentiated from extrafusal fibers; nuclear chain fibers did not form. The capsules of spindles disassembled, hence spindles or their remnants could no longer be identified 1 week after denervation. Neonatal deefferentation has little effect on these features of developing spindles, so removal of afferent innervation is presumably the factor that induces the loss of spindles in denervated muscles. Degeneration of the bag2 fiber, but not bag1 or extrafusal fibers, reflects a greater dependence of the bag2 fiber than the bag1 fiber on afferent innervation for maintenance of its structural integrity. This difference in response of the two types of immature bag fiber to denervation might reflect an origin of the bag2 fibers from a lineage of myogenic cells distinct from that giving rise to bag1 or extrafusal fibers, or a difference in the length of contact with afferents between the two types of bag fiber prior to nerve section.

Rat muscle spindles contain one nuclear bag2, one nuclear bag1, and two nuclear chain fibers. The three different types of intrafusal fiber in spindles may be a reflection of concomitant changes in proportions of slow primary, slow/fast secondary, and fast secondary myotubes during the period of spindle development. We examined whether experimentally altering the available muscle substrates would impact the intrafusal fiber type composition of spindles. De novo formation of spindles in muscles devoid of primary myotubes was induced by crushing the nerve to the medial gastrocnemius muscle in newborn rats and administering nerve growth factor for ten days afterwards. Encapsulated fibers of the reinnervated muscles examined one month after nerve crush had myofibrillar adenosine triphosphatase and myosin heavy chain profiles similar to normal bag2, bag1, or chain intrafusal fibers. However, spindles in reinnervated muscles contained fewer fibers than controls. Most experimental spindles contained chain and/or bag1 fibers, the two fiber types which ordinarily arise during secondary myogenesis. In contrast, bag2 fibers, fibers that normally form concomitant with primary myogenesis, were absent from nearly 90% of spindles in reinnervated muscles. The paucity of bag2 fibers may reflect the absence of primary myotubes, whereas the prevalence of chain and/or bag1 fibers may reflect that secondary myotubes or myofibers that descended from the secondary myotubes were the principal muscle substrates available for spindle formation in the nerve-crushed muscles. The paucity of bag2 fibers in spindles formed in muscles devoid of primary myotubes suggests that the types of muscle substrates available to afferents are an important determinant of intrafusal fiber types in muscle spindles, and that the formation of a bag2 fiber in an intrafusal bundle is not essential for the subsequent differentiation of chain and/or bag1 fibers.

Crushing the nerve to the medial gastrocnemius muscle in newborn rats and administering nerve growth factor afterwards results in a reinnervated muscle containing supernumerary muscle spindles. The structure and innervation of 88 spindles in the reinnervated muscles were reconstructed from serial thick and thin transverse sections at 30 35 days after the nerve crush, and compared to those of five control spindles. The spindles consisted of one to four small-diameter encapsulated fibers with features of nuclear chain intrafusal fibers, or infrequently a nuclear bag intrafusal fiber. Some of the spindles were located within a capsule that also contained an extrafusal fiber. Each spindle was innervated by an afferent with features of the primary afferent. The density of secondary afferents was lower in reinnervated muscles than in controls. Endplates were observed on extrafusal fibers in the experimental muscles. attesting to restoration of skeletomotor (alpha) innervation after the nerve crush. However, 78% of the experimental spindles were entirely devoid of efferent innervation. The remainder received either one or two fusimotor (gamma) axons or a skeletofusimotor (beta) axon, compared to the six to eight motor axons that innervated control spindles. The presence of supernumerary spindles composed of fibers that resemble normal intrafusal fibers in the absence of motor innervation suggests that afferents alone can induce the formation and subsequent differentiation of intrafusal fibers in nerve-crushed muscles of neonatal rats. In addition, the paucity of gamma innervation in nerve-crushed muscles suggests that immature gamma neurons are more susceptible than spindle afferents or alpha efferents to cell death after axotomy at birth.

ALD 19, a monoclonal antibody that recognizes the slow-tonic myosin heavy chain (MHC) isoform, has been used extensively as a marker for nuclear bag intrafusal fibers of muscle spindles in developing and adult rats. Extrafusal fibers of adult rat hindlimb muscles do not express slow-tonic MHC. However, while using ALD 19 to trace the fate of intrafusal fibers following neonatal denervation, we noted that some extrafusal fibers of neonates also bound this antibody. The immunolabeled extrafusal fibers were a subset of slow fibers located in the deep axial regions of crural muscles. The same fiber subset transiently displayed a weak affinity for ALD 19 during the first postnatal week in normal muscles. Denervation at birth increased the intensity of ALD 19 immunolabelling by these extrafusal fibers and extended the duration of the slow-tonic immunoreactivity into the 2nd postnatal week, after which expression diminished or ceased. Demonstration that some developing extrafusal fibers have a nerve-independent capacity for transiently expressing slow-tonic MHC, an MHC previously thought to be expressed only by intrafusal fibers, raises the possibility that both types of fiber originate from a subset of bipotential slow primary myotubes in rat hindlimbs.

Objectives.-To evaluate if caregivers are reliable informants concerning memory deficits in patients with Alzheimer's disease (AD). Design.-Responses of caregivers of patients with probable AD and responses of healthy control subjects on a standardized memory questionnaire were compared with objective measures of cognition (Consortium to Establish a Registry for Alzheimer's Disease neuropsychological battery) and with clinical estimates of activities of daily living, depression, and psychopathology (Consortium to Establish a Registry for Alzheimer's Disease [CERAD] clinical assessment battery) using the Self-report Memory Questionnaire. Setting.-A federally funded AD research center. Subjects.-The referred sample included 117 patients with probable AD, their informants, and 41 healthy control subjects age-matched to the patients. Patients and control subjects were between the ages of 58 and 85 years, had between 9 and 19 years of education, and were in good health. Exclusions.-Patients who did not meet NINCDS-ADRDA criteria of probable AD. Main Outcome Measure.-The optimal number of questionnaire items yielding the best combination of sensitivity and specificity. Results.-An abbreviated version of the scale, renamed the Short-Memory Questionnaire, had excellent specificity and sensitivity for identifying dementia. Positive and negative predictive values were 63.5% and near 100%, respectively. The Short-Memory Questionnaire showed good reliability, internal consistency, and external validity. Caregiver appraisals of memory deficits significantly correlated with objective measures of memory and also with generalized cognitive dysfunction. Conclusions.-Caregivers of patients with AD are reliable informants of their relatives' deficits. The Short-Memory Questionnaire is an easily administered, informant-based scale that may be useful in clinical settings or epidemiologic studies to screen out persons with memory difficulties.

As the incidence of human immunodeficiency virus (HIV) infection increases, health care practitioners are being called upon to counsel and educate patients regarding the complications associated with HIV infection. In addition, the anxiety levels of health care workers reveal that similar educational efforts must be initiated early in the training of health care professionals. In this study we surveyed 341 first and second year medical students from three medical schools to assess their levels of knowledge about how HIV infection is and is not transmitted. Results were analyzed overall, by site, by gender, and by the respondents' locality (urban or rural). Analysis revealed that knowledge regarding HIV transmission varied dramatically among these first and second year medical students. Place of residence, gender, and class rank did not appear to be factors affecting these students' knowledge. We recommend that caution be exercised in assuming that prospective physicians possess current knowledge on how HIV infection is transmitted.

The distribution of nitric oxide synthase (NOS) in the mouse olfactory bulb and olfactory epithelium, including the vomeronasal organ, was studied using an anti-NOS antibody, NADPH diaphorase histochemistry and in situ hybridization with NOS specific antisense oligonucleotide probes. Interneurons containing NOS protein and mRNA, and exhibiting NADPH diaphorase activity were detected in the plexiform layer of the main olfactory bulb and the granule cell layer of main and accessory olfactory bulbs. Periglomerular cells and granule cells in the main olfactory bulb were also NOS positive with diaphorase and immunostaining for NOS. In contrast, no evidence for NOS expression was found either in the main olfactory epithelium or in the vomeronasal organ, in spite of the strong diaphorase staining of the surface of the main olfactory epithelium. Polymerase chain reaction amplification experiments for detection of NOS gene expression further indicated that NOS is expressed in the olfactory bulb but not in either the main olfactory epithelium or vomeronasal organ. Use of an antibody raised against another enzyme, NADPH-P450 oxidoreductase, showed that this protein was strongly expressed in the olfactory epithelium, Activity of this enzyme may account for the diaphorase histochemical staining of the epithelia. An involvement of neuronal nitric oxide synthase in signalling in olfactory receptor neurons is therefore doubtful, although NOS is clearly expressed in neurons in both main and accessory olfactory bulbs.

Cholesterol 7alpha-hydroxylase, the rate-determining enzyme in the bile acid biosynthesis pathway, is regulated in a negative feedback manner by hydrophobic bile salts returning to the liver via the portal circulation. The role of cholesterol in the regulation of cholesterol 7alpha-hydroxylase and the interrelationship between the cholesterol and bile acid biosynthesis pathways remain controversial. The objective of the present study was to define the role of cholesterol in the regulation of cholesterol 7alpha-hydroxylase and determine the molecular level of its control. In order to avoid intestinal or intravenous administration of cholesterol, we manipulated the flow of cholesterol within the hepatocytes by decreasing cholesterol synthesis with lovastatin in bile fistula rats (bile acid synthesis is up-regulated), or by increasing cholesterol supply by administering mevalonate, a precursor of cholesterol, to rats with intact enterohepatic circulation (bile acid synthesis is normal). In the first series of studies, lovastatin was administered as a single intravenous bolus (10 mg/kg) to rats with chronic bile fistula and to rats with intact enterohepatic circulation (cholesterol and bile acid synthesis is normal). Three hours after lovastatin administration, cholesterol 7alpha-hydroxylase specific activity, enzyme mass, mRNA, and gene transcriptional activity were decreased by 35%, 32%, 56%, and 34%, respectively, in rats with chronic bile fistula. In rats with intact enterohepatic circulation, lovastatin administration resulted in a similar decrease (34%) of cholesterol 7alpha-hydroxylase specific activity. In the second group of experiments, rats with intact enterohepatic circulation were administered a 180 mum bolus of mevalonate followed by a continuous infusion of 180 mumol/h for 1.5, 3, 4.5, and 24 h prior to being killed. Continuous infusion of mevalonate increased cholesterol 7-alpha-hydroxylase specific activity, mRNA levels, and transcriptional activity by an average of 2- to 3-fold at all time intervals. We conclude that under circumstances in which cholesterol is present in excess, cholesterol 7alpha-hydroxylase transcriptional activity is up-regulated and removal of cholesterol from the hepatocytes is facilitated by an increase of bile acid synthesis. When cholesterol availability is decreased, cholesterol 7alpha-hydroxylase transcriptional activity is down-regulated leading to a decreased elimination of cholesterol via bile acid synthesis. In both instances, hepatic cholesterol homeostasis is effectively maintained.

Various indirect forms of evidence are provided for the accuracy of synthetic estimation of the distribution of severely mentally disabled (SMD) adults across Ohio's mental health board areas. The board areas are public mental health jurisdictions which are allocated state funds to serve the needs of SMD adults. Equitable allocation of funds to these areas depends on accurate estimation of the distribution of SMD adults across the areas. The synthetic estimation method involved two steps. (a) ECA-based prevalences of SMD adults by demographic profiles were obtained, (b) prevalences were applied to a board area's demographic mix to yield the estimated SMD count for that area. The main indirect evidence of accuracy showed that the synthetically-estimated percentages across areas agreed closely with the actual percentages of SMD adults enrolled in federal programs. Statistical characteristics of various percentage estimators indicated that reasonably accurate estimation and allocation is not difficult to attain.

Background: Numerous studies have reported differences in cancer staging at diagnosis and in survival between Black and White patients with breast cancer. Utilizing data obtained from the National Cancer Institute's (NCI's) Black/White Cancer Survival Study for the period 1985-1986, a new study is presented here that systematically examines multiple explanatory factors (e.g., lack of mammograms) associated with these cancer-staging differences. Purpose: We evaluated within a single study the relationship of selected demographic, lifestyle, antecedent medical experiences, and health care acces s factors to cancer staging at diagnosis in Black and White breast cancer patients. Methods: Data utilized in this population-based cohort study of 1222 eligible women (649 Black and 573 White) newly diagnosed for the period 1985-1986 with histologically confirmed primary breast cancer were obtained from the NCI's Black/White Cancer Survival Study. Sources of data included abstracts of hospital medical records, central review of histology slides by a study consultant pathologist, and patient interviews obtained from three metropolitan areas: Atlanta, New Orleans, and San Francisco-Oakland. Within each area, 70% of all Black incident cases were randomly selected, and a sample of White cases, frequency matched by age groups (20-49 years, 50-64 years, and 65-79 years), was selected for comparison. Stage of breast cancer at diagnosis was classified according to the international tumor-lymph node-metastases (TNM) system. Statistical models utilized in this study included the log-linear and polychotomous logistic regression with multiple predictor variables. Results: Factors associated with cancer staging were differentially expressed in Blacks and Whites. Indicators of access to health care, a lack of mammograms, and an increased body mass index significantly (P<.02) contributed to stage differences in Blacks, whereas income was marginally associated (P = .06) with stage for Whites only. Nuclear grade, having a breast examination by a physician, and a history of patient delay explained approximately 50% of the excess risk for stage III-IV Cancer versus stage I-II(N0) cancer among Blacks compared with Whites (odds ratio- reduction from 2.19 to 1.68). Conclusion: These findings suggest that no single factor or group of factors can explain more than half of the race-stage differences noted in this study with respect to Black and White breast cancer patients.

The records of 167 patients who underwent both cone biopsy and hysterectomy at the University of Michigan Medical Center between 1974 and 1989 were reviewed. The presence or absence of neoplasia at the cone biopsy margins was able to be correlated with the extent of residual disease in the hysterectomy specimens for 138 patients. When all patients were evaluated, residual disease was found in 64 of 82 cases (78%) when the cone biopsy margins were involved and in 14 of 56 cases (25%) when the cone margins were free. Among patients with preinvasive disease on cone biopsy, 37 of 50 (74%) had residual disease when the cone margins were involved and 11 of 48 (23%) had residual disease when the cone margins were free. No patient with mild to moderate dysplasia on cone biopsy had residual disease more advanced than moderate dysplasia, regardless of margin status. When the margins were free on cone biopsy, there was no residual disease in 12 of 15 patients (80%) with severe dysplasia and no residual disease in 18 of 22 patients (82%) with CIS. These findings suggest that patients with mild to moderate dysplasia regardless of cone margins, severe dysplasia, and CIS with free cone margins have a low rate of residual disease remaining in the cervix.

Sexual behavior, knowledge of HIV transmission and prevention, perceived risk of AIDS, and safer sex behavior were studied in a sample of 289 single, pregnant. inner-city women. African-American and European-American women were equally represented. Women had poor AIDS knowledge. Sexual behavior placed women at risk for HIV infection due to the lack of condom or spermicide use. Women did not perceive themselves at risk for the AIDS virus, although they did recognize that heterosexuals were at risk. Their lack of risk perception was partly based on their having a single sexual partner. They did not regard their partner's current or past behavior as placing them at risk. Recommendations for intervention and cultural differences were discussed.

Radiographic confirmation of enteral feeding tube placement is a common practice representing considerable expense and causing delay in the initiation of enteral nutrition therapy. We evaluated an enteral feeding tube with a pH sensor, which allows immediate verification of the location of the tube by assessment of the pH upon insertion. Insertion pHs were obtained for 24 intensive care unit patients requiring feeding tube placement. Placement was verified radiographically and compared with expected location on the basis of the pH profile. The radiograph and the insertion pH profile were in agreement in 87.5% (21 of 24) of the cases. Concomitant use of histamine blockers did not affect the ability of the pH sensor to detect placement accurately (Fisher's Exact Test, p 5 .71) Use of these pH measurements eliminates the need for radiographic documentation of placement, provides a savings for the patient, and may be beneficial in promoting enteral feedings in critically ill patients.

A fast-eluting minor variant of hemoglobin A, designated as HbA1-AcH, appears elevated after the incubation of red blood cell hemolysates with acetaldehyde (AcH) and has been proposed as a diagnostic marker for alcoholism or as an indicator for heavy drinking. We have developed an improved HPLC separation of this peak and others elevated by AcH using a polyaspartic acid column (PolyCAT A, PolyLC, Inc.) and a nonlinear buffer gradient with pH changes from 6.6 to 6.8. Saline-washed red blood cells were treated with sodium acetate buffer (pH 5.5) to remove unstable Schiff bases, and then hemolyzed by addition of an equal volume of H2O and 0.4 volumes of CCl4. HbA1-AcH and several others, including two peaks in the HbA1a+b cluster, Hb Pre-A1c, and HbA1d3 were significantly increased by AcH incubation, and the changes were only partially reversible with time. Improved resolution of these peaks allows more accurate quantitation of AcH adducts of hemoglobin.

1. Activation of GABA(A) receptors can produce both hyperpolarizing and depolarizing responses in CA1 pyramidal cells. The hyperpolarizing response is mediated by a Cl- conductance, but the ionic basis of the depolarizing response is not clear. We compared the GABA(A) receptor-mediated depolarizations induced by synaptically released gamma-aminobutyric acid [GABA; depolarizing inhibitory postsynaptic potentials (dIPSPs)] with those produced by exogenous GABA (depolarizing GABA responses). Short trains of high-frequency (200 Hz) stimuli were used to generate dIPSPs. We found that dIPSPs generated by trains of stimuli and depolarizing responses to exogenous GABA were accompanied by a conductance increase and had a similar reversal potential, indicating a similar ionic basis for both responses. 2. We wished to determine whether an HCO3- current contributed to the GABA(A)-mediated depolarizations. We found that dIPSPs and depolarizing GABA responses were sensitive to perfusion with HCO3--free medium. Interpretation of these data was complicated by the mixed nature of the responses: dIPSPs were invariably accompanied by conventional, Cl--mediated fast hyperpolarizing IPSPs (fIPSPs), and response to exogenous GABA usually consisted of biphasic hyperpolarizing and depolarizing responses. However, it was sometimes possible to elicit responses to GABA that appeared purely depolarizing (monophasic depolarizing GABA responses). 3. We analyzed monophasic depolarizing GABA responses and found no change in reversal potential when slices were perfused with HCO3--free medium. We also made whole-cell recordings from CA1 pyramidal cells, attempting to reduce [HCO3-]i, and compared the reversal potential for monophasic depolarizing GABA responses with similar responses recorded with fine intracellular microelectrodes. We found no difference in reversal potential. We also examined effects of the carbonic anhydrase inhibitor acetazolamide (ACTZ) on depolarizing GABA responses. ACTZ reduced these responses but did not change their reversal potential. 4. Effects of HCO3--free medium were not specific to GABA(A) receptor-mediated responses. GABA(B) receptor-mediated slow IPSPs (sIPSPs) were also reduced, as were excitatory postsynaptic potentials (EPSPs). Analyses of field potentials and spontaneous fIPSPs suggested a decrease in presynaptic excitability during perfusion with HCO3--free medium. In addition, pyramidal cells showed decreased input resistance when perfused with HCO 3--free medium. 5. The sensitivity of GABA(A) receptor-mediated depolarizations to HCO3--free medium can be explained by a decrease in presynaptic excitability and an increased resting conductance in postsynaptic neurons. Reduced presynaptic excitability and resting input resistance are also likely causes of the reduction in fast IPSPs, slow IPSPs, and EPSPs in HCO3--free medium. We suggest that these nonspecific effects of HCO3--free medium may be a consequence of an extracellular acidification. These data do not provide convincing evidence for involvement of an HCO3- conductance in the generation of dIPSPs and depolarizing GABA responses.
