Far-advanced otosclerosis (FAO) is an uncommon diagnosis. Hearing levels in patients with FAO may range from profound loss, by air conduction and fragmentary bone conduction thresholds, to no measurable air or bone conduction thresholds. Thus, FAO may be difficult to distinguish from a sensorineural hearing loss. This report presents the results of surgery in 73 ears with FAO, 77 percent of which had improvement in air conduction thresholds of greater than 20 dB. Discrimination was improved by more than 15 percent in 54 percent of cases, and 75 percent realized improvement in use of a hearing aid. There was no evidence that success was related to preoperative hearing. The surgical results of a subgroup of 14 patients having bilateral FAO were also analyzed. For all 14, similar surgical outcomes were achieved in both the initial and the contralateral ear, with six successes bilaterally and eight failures bilaterally. Although far advanced otosclerosis is uncommon and difficult to diagnose, surgery is worthwhile.

Inbred mice of both sexes of the CBA/KI, C3H/f/KI, and C57 Black/KI strains were fed seven different commercially-available brands of bee pollen and drinking water ad libitum, starting at young adult age (30-35 days), as the only source of food intake for periods up to one year. Ninety-eight of 100 mice or the three strains survived in a healthy condition (360 to 369 days) when fed four of the seven brands studied. Zero of 80 mice of the CBA/Kl and C3H/f/KI strains survived when fed the other three brands, with all mice dying over a period of four to 25 days after starting on the bee pollen diet. It was not possible to determine whether these differences in the brands of bee pollen were related to differences in quality or quantity of nutritional value or the use of potentially toxic additives or binders to mice in the packaging of the bee pollen product. A second study examined the growth of CBA/KI male and female mice fed a bee pollen diet between 30 and 90 days of age. The bee pollen diet selected for this study consisted of natural bee pollen granules proven to support long-term survival of CBA/KI mice in the first study. The study compared body weight, nose-rump length, and organ weights including those of the brain, heart, lungs, liver, gut, kidney, spleen, gastrocnemius muscle and inguinal/gonadal fat depots of the pollen fed mice to control CBA/KI mice fed a standard laboratory mouse chow. There were no significant differences in either males or females of the two dietary groups when comparing wet weights of brains, hearts, lungs, livers or kidneys. There were two strong differences between the two groups. On one hand, the gut weights in the bee pollen fed mice were significantly heavier, and on the other hand, the spleen and fat depot weights were significantly lighter in the bee pollen groups. These findings demonstrate that certain commercially available brands of bee pollen contain all the necessary nutritional elements to maintain laboratory mice in a healthy condition for a period of at least one year as well as provide the essential quality and quantity of nutritional factors to promote comparable body and organ growth during the period between 30 to 90 days of age. These findings also suggest it might be practical to use bee pollen as part of a nutritional maintenance program for different mammals. Bee pollen may also help meet part of human nutritional needs.

A rat muscle spindle typically contains four intrafusal fibers - one nuclear bag(2), one nuclear bag, and two nuclear chain fibers. We compared the sequence of formation of the three intrafusal fiber types among the tibialis anterior (TA), soleus (SOL) and medial gastrocnemius (RIG) muscles using immunocytochemistry of spindle-specific myosin heavy chain isoforms. Spindles of the TA began to differentiate earlier and acquired the full complement of intrafusal fibers sooner than spindles of the SOL or MG muscles. At the onset of spindle assembly, the intrafusal myotubes expressed myosin heavy chains similar to those expressed by extrafusal myotubes. The first intrafusal myotube then differentiated into the bag, fiber regardless of the muscle. However, the fate of the second-forming intrafusal myotube varied among the muscles studied. It usually differentiated into a chain fiber in the TA, into a bag(1) fiber in the SOL, and into either a bag(1) or a chain in the MG. The fate of the third-forminge was reciprocal to that of the second; i.e. in those spindles in which the bag(1) fiber was second to form, a chain was third, and vice versa. The fourth and last intrafusal myotube gave rise to a chain fiber. The inter- and intramuscular variability in the fate of intrafusal myotubes of the second and third generation argues against the existence of a program intrinsic to the myotubes that would mandate their differentiation along specific paths. Rather, an extrinsic regulatory factor, probably associated with the primary afferent neuron, may govern differentiation of pluripotential myotubes into particular types of intrafusal fiber. The fate of the intrafusal myotubes might then depend on the timing of the regulatory effect of afferents relative to the stage of development of the intrafusal bundle.

The factor structure of the Family-of-Origin Scale (FOS) was examined. The FOS is a 40-item instrument devised to measure perceptions of health in the family of origin. Subjects were 162 gay and bisexual males. A confirmatory factor analysis was employed to examine the similarities of the factor structure obtained in the present study with those proposed theoretically by the test authors. The results supported the notion that the FOS is comprised of a single underlying construct that may be represented by bipolar items. It was concluded that the findings do not support the theoretically proposed factor structure of the FOS and that the value of the FOS is suspect.

In the present study we employed current source density analysis to study the major excitatory/inhibitory pathways in rat primary visual cortex in vivo and in vitro. A natural photic stimulus was used in vivo and served as a baseline for understanding the results obtained from in vivo and in vitro studies employing electrical stimulation of the white matter. The temporal pattern of synaptic activity in the cortex revealed an early excitation, characterized by sinks of short duration and high amplitude, that was followed by inhibition, characterized by long lasting, low amplitude active sources. The spatial pattern of this synaptic activity displayed early excitatory inputs to layer IV and lower layer III. Supragranular layers exhibited synaptic activity of longer latency at more superficial layers. The excitatory activity of the infragranular layers was delayed relative to that in layer IV. This spatial and temporal pattern of synaptic activity supports the model of sequential information processing in visual cortex. Based on the results of electrical and photic stimulations in vivo we conclude that electrical stimulation of white matter activate the thalamo-cortical input which results in a similar laminar pattern of postsynaptic activity evoked by photic stimulation. Electrical stimulation revealed additional antidromic and anti-orthodromic activity (collaterals of descending axons to white matter), resulting in the early fast components and the additional activity in layer VI. The major differences between in vivo and in vitro laminar pattern of synaptic activity (applying electrical stimulation) were reduced synaptic activity in layer IV and increased synaptic activity in the infragranular layers in the in vitro preparation. We concluded that the visual cortex slice preparation preserves the major pathways and electrophysiological function of this area. The technical advantages of the cortical slice preparation will facilitate studies and provide additional insight into this complex cortical network.

The management of traumatic biliary fistulas is controversial. New ideas in nonsurgical treatment have recently evolved, in part because of the rapid advancement of laporoscopic surgery. Three major concepts are important in managing biliary fistulas: diagnosis, drainage, and decompression. These concepts were applied to a trauma patient, then reviewed in detail.

We examined women's barriers to safer sex and the development of a 14-item, multi-dimensional, barriers to safer sex scale. In Study 1, model testing and scale development was conducted on a population of 503 African and European American, inner-city, pregnant, single women. We found four factors that closely parallel theoretical barriers that are cited in the literature: a) partner and self objections, b) the appraisal that one is not at risk, c) embarrassment, and d) giving up pleasure. In Study 2, convergent and divergent validity data supported the construct validity of the theoretical model and scale. In Study 3, the four subscales were found to be reliable among an additional sample of 72 single, college women. Subscale means for the community and student samples differed significantly, indicating important population differences. More modest differences were found between African American and European American women.

Behavioral change reduces risk of HIV infection and development of AIDS. We compared 206 inner-city women who were randomly assigned to a 4-session AIDS-prevention group or to one of two controls, a health-promotion group or a no-intervention group. AIDS-prevention and health-promotion groups provided information, behavioral competency training, and social support. Only the AIDS-prevention group focused on AIDS-specific knowledge and skills. The AIDS-prevention group produced moderate, consistent increases in knowledge and safer sex behaviors in comparison with either the health-promotion or no-intervention group. Self-report and objective changes were sustained 6 months after intervention for both African-American and European-American women.

Objective: Investigation of the hypothesis that the infusion of 10 ng/kg/min prostacyclin (epoprostenol) (PGI(2)) improves O-2 uptake in patients with hyperdynamic septic shock. Design: Prospective, single cohort design. Setting: ICU, university hospital. Patients: Fifteen postoperative patients with septic shock. Interventions: Infusion of 10 ng/kg/min of PGI(2) for 60 min. Measurements: Complete hemodynamic profile with O-2 transport-related variables (simultaneous measurements of Vo(2) from the respiratory gases and by cardiovascular Fick) and blood lactate levels before start of the PGI(2)-infusion and 60 min thereafter. Main results: Oxygen delivery increased significantly (14 percent) from its already high value, 750+/-238 to 852+/-214 ml/min/m(2). The O-2 extraction ratio remained unchanged. When Vo(2) was measured from the respiratory gases, it was unchanged. When Vo(2) was measured by cardiovascular Fick, it increased slightly (p<0.05). Conclusions: We conclude that in this O-2 challenge test with PGI(2) in patients with septic shock, an increase in O-2 delivery was not matched by an increase in Vo(2). We believe that the adequate conventional support of these patients may have prevented the PGI(2) from revealing a ''covert'' O-2 debt. The PGI(2) test did not predict mortality by O-2 supply dependency. The small increase in Vo(2) as calculated indirectly suggests a degree of mathematical coupling of O-2 delivery and uptake.

OBJECTIVE: To describe a patient who developed significant metabolic abnormalities in response to a low-level theophylline ingestion. CASE SUMMARY: An 18-year-old man was examined after ingesting theophylline 3 g in a suicide attempt. Although his peak theophylline concentration was 157 mumol/L (28.2 mug/mL), it was associated with significant leukocytosis, hypokalemia, hypomagnesemia, hypophosphatemia, hyperglycemia, and lactic acidosis. These abnormalities have been previously associated with theophylline intoxication, but only in conjunction with much higher peak concentrations of theophylline. CONCLUSIONS: Significant metabolic abnormalities can occur with suicidal ingestion of relatively small amounts of theophylline. The presence of these abnormalities should be sought in theophylline overdoses. In the proper clinical circumstances, such abnormalities should raise suspicion of covert theophylline ingestion.

N-methyl-D-aspartate (NMDA) receptors are important in many instances of synaptic plasticity. In hippocampal area CA1, long-term potentiation (LTP) can be induced by both NMDA receptor-dependent and -independent mechanisms. Using intracellular recordings and single-electrode voltage clamp, we isolated and characterized NMDA receptor-mediated synaptic responses. NMDA receptor-mediated responses evoked by low frequency orthodromic stimulation were inhibited in a dose-dependent manner by the competitive antagonist D,L-2-amino-5-phosphonovaleric acid (APV). High frequency (tetanic) stimulation, which facilitates synaptic release of glutamate, failed to overcome the blockade of NMDA receptors by APV. Using extracellular recordings of field potentials, we studied the contribution of NMDA receptors to LTP induced by different patterns of tetanic stimulation. LTP was inhibited in a dose-dependent manner by APV, but was more sensitive to APV than were NMDA receptor-mediated synaptic responses. This most likely reflects a threshold for NMDA receptor activation in LTP induction. A component of LTP that resisted blockade by APV was induced by high (200 Hz), but not low (25 Hz), frequency tetanization. This NMDA receptor-independent component of LTP persisted for > 4 hours and accounted for approximately half the potentiation induced by 200 Hz tetanization. Procedures necessary to induce LTP at the Schaffer collateral/commissural synapses in area CA1 by both NMDA receptor-dependent and -independent mechanisms are now well characterized. An account of the resource
Objective: To determine the efficacy of treatment of significant hirsutism with a GnRH agonist (GnRH-a) and estrogen and progestin replacement therapy. Design: Clinical series. Setting: Ambulatory gynecology clinic in a community hospital. Patients: Ten women with significant hirsutism caused by polycystic ovarian disease. Interventions: The patients were treated with leuprolide acetate 20 mu g/kg per day in combination with E(2) (2 mg) and medroxyprogesterone acetate (5 mg) for 6 months. Main Outcome Measure: Hirsutism scores and hair growth rates determined before and upon completion of treatment protocol. Results: Hirsutism scores and hair growth rates significantly decreased by 23% and 26%, respectively. The duration of hirsutism was the only significant covariate for hirsutism scores and hair growth rates. Only two patients had minimal, irregular bleeding that was corrected by increasing the estrogen dose. Conclusion: The combination of a GnRH-a and estrogen replacement therapy was an effective and well-tolerated treatment in a small group of women with significant hirsutism caused by PCOD.

Human papillomavirus, a papovavirus, is the causative agent of all warts. There are at least 63 subtypes of this virus, certain ones of which are more likely to cause warts in particular locations or to be associated with malignancies. Over the last 6 years, HPV-6, HPV-11, and HPV-16 have been detected in both benign and malignant lesions of the conjunctiva. We report a patient with a large viral papilloma of the palpebral conjunctiva of the right lower eyelid. This case demonstrates that even very large viral papillomas can be treated successfully.

In Experiment I, castrated male and female CD-1 mice +/- estradiol benzoate (EB) treatment were tested for their performance on a skilled sensorimotor task consisting of walking across beams of varying widths (6, 9, 12, and 21 mm). To evaluate whether behavioral performance was related to nigrostriatal dopaminergic function, tissue dopamine concentrations were determined from the corpus striatum as well as the hypothalamus and olfactory tubercle. In general, sensorimotor performance improved for all treatment conditions as the beam width increased. Castrated male mice treated with oil vehicle showed the worst performance as indicated by significantly greater amounts of time to cross the beam. Treatment of castrated males with EB significantly improved their performance. Performance of the castrated females was not changed by EB treatment and was similar to that observed with the castrated + EB males. Significant gender differences in dopamine concentrations (female > male) were obtained in the corpus striatum, as well as the olfactory tubercle and hypothalamus. Dopamine levels were unaltered by EB treatment. In Experiment II, behavioral and neurochemical determinations were directly compared between castrated and intact male mice. Behavioral performance of castrates was significantly reduced compared to intact males. No differences in dopamine concentrations were obtained between these two groups; however, the hypothalamic dopamine/DOPAC ratio of castrates was significantly greater than that of intact male mice. These results demonstrate significant modulatory effects of EB in castrated male, but not female, mice upon sensorimotor performance and indicate that this task may provide an effective means to partial out modulatory effects of gonadal steroid hormones upon skilled sensorimotor performance. When the data of Experiments I and II are combined, it appears that the basis of this sensorimotor deficit in the males is the absence of gonadal steroid hormones. In addition, these results reveal significant gender differences in various dopaminergic systems in these mice. (C) 1994 Academic Press, Inc.

OBJECTIVE: To identify educational strategies for resident training in nursing home care deemed successful by a large number of programs. DESIGN: A mail survey with three follow-up mailings. PARTICIPANTS: Directors of accredited internal medicine and family practice residency programs. MEASUREMENTS: Open- and closed-ended questionnaire eliciting curricular content, instructional strategies, and evaluation techniques from programs offering a nursing home experience. Identification of barriers to implementation of a nursing home curriculum and recommendations for success were requested. MAIN RESULTS: Of the 814 surveys mailed, 537 were returned for a response rate of 66%. Nursing home experiences were required in 86% of family practice residency programs but in only 25% of internal medicine programs. Most geriatric medicine curricular content areas were taught in the nursing home; however, relatively little emphasis was given to rehabilitation, organization, and financing of health care, and coordination of care between acute and chronic settings. Direct patient care, bedside rounds, and lectures were the most common instructional strategies reported. Evaluation approaches included faculty observations, resident attendance, and chart reviews with written and skill based examinations infrequent. Availability of faculty and conflict with other rotations were identified as the principal barriers to implementation of nursing home rotations. An organized nursing home curriculum supervised by enthusiastic faculty using a longitudinal rotation format with resident involvement in an interdisciplinary team was recommended. CONCLUSIONS: Educational strategies exist for successful implementation of a residency nursing home curriculum. Greater priority must be given to training residents in nursing home care and developing nursing home faculty to substantially increase the number and quality of physicians who practice in this setting.
