False-positive Uptake Of Fdg In Hepatic Sarcoidosis

Positron emission tomography (PET), with the use of F-18 fluorodeoxyglucose (FDG), has been shown to be useful in the evaluation of many tumors as a result of its high sensitivity and specificity. There are instances, however, in which false-positive FDG uptake can occur with inflammatory lesions or granulomatous diseases. In fact, false-positive mediastinal uptake in thoracic sarcoidosis has been well documented. We present a case of false-positive FDG uptake in the liver resulting from hepatic sarcoidosis. A 42-year-old, asymptomatic woman underwent a PET scan after a solitary pulmonary nodule was detected on a chest CT. The nodule in the left lower lobe on CT revealed no metabolic activity, but 2 lesions were found in the liver with SUVs in the range of 2.5 to 3.2. A subsequent MRI was not able to exclude malignancy. The liver lesions were surgically removed and found to be sarcoidosis.

Diagnosis of pneumothorax on F-18 FDG PET after transbronchial biopsy.

Transbronchial lung biopsy (TBLB) is associated with pneumothorax, 12% occurring 1 to 4 hours postprocedure (delayed pneumothorax). We report a case of a delayed pneumothorax first diagnosed on a positron emission tomography (PET) scan performed 5 days after TBLB. An early post-TBLB chest radiograph (after 1 hour) had not demonstrated any pneumothorax. The delayed pneumothorax was later confirmed with a repeat radiograph after the PET scan. Often, PET is performed to diagnose or stage pulmonary lesions after TBLB rather than before. Therefore, nuclear physicians must remain vigilant to recognize subtle pneumothoraces on PET scans.

Functioning liver metastases on an I-131 whole-body scan: a case of malignant struma ovarii.

A 46-year-old women was examined for severe constipation. Pelvic examination revealed a large pelvic mass extending to the level of the umbilicus. Computed tomography showed a large multicystic, septated mass in the pelvis and a small amount of fluid in the cul de sac. In addition, multiple ill-defined, mixed-attenuation hepatic lesions were identified. A malignant ovarian neoplasm with liver metastases was considered, so the pelvic mass was resected. Interestingly, histopathologic analysis revealed malignant struma ovarii of the follicular type. Biopsy of the liver lesions confirmed metastatic disease with similar histopathologic findings. All thyroid laboratory values were in the normal range. The patient then had a total thyroidectomy to optimize thyroid ablation therapy with I-131. This revealed a small follicular adenoma but no evidence of cancer. An I-131 whole-body scan was performed and showed uptake in multiple functioning liver metastases.
