Description
This article provides an update and overview to the clinical presentations of inflammatory arthropathies. Subtleties to clinical presentations are discussed. Clues are presented which helps the reader arrive at more precise diagnostic labeling. Additionally, pharmacotherapy will be discussed, including precautions in considering the best therapy for the patient with suspected inflammatory, autoimmune, degenerative or neuropathic pain conditions. Finally, preoperative evaluations, management and risks of this patient population are reviewed. Emphasis will be on whether "cardiac clearance" should be requested based on an easy to use algorithm of cardiac risk factors. Finally recommendations based on recent literature of whether immunosuppressants should be withheld preoperatively.

Description
Staphylococcus and streptococcus are bacteria commonly found on the skin and in the mucous membrane. These bacteria are frequently associated with mild cutaneous infections but may invade to cause deeper infections including necrotizing soft-tissue infections, sepsis with or without metastatic lesions, and immunologic or toxin associated sequelae. Early recognition and timely intervention are the hallmarks of appropriate management. Initial empiric therapy should be directed towards a broad spectrum of pathogens that include staphylococcus and streptococcus.

Description
Chronic wounds of the lower extremity can pose several obstacles for the treating physician as well as the patient. Peroneal artery adipofascial flaps have been shown to be a reliable and durable option for reconstruction of these defects for many reasons, including ease of dissection, minimized donor site mobility, reliability of take to underlying surfaces, and cosmetically more pleasing outcome. This article discusses the current literature regarding the use of peroneal artery adipofascial flaps and our surgical technique for harvest, transfer, and postoperative course for this versatile flap.
