An account of the resource
Abnormalities that increase water reabsorption can lead to hyponatremia, and the resulting water retention presents the threat of CNS volume changes. Avoid overcorrection of hyponatremia, however, as this causes similar changes. Also, be aware of iatrogenic development of hypotonicity in groups of patients prone to hyponatremia. Careful administration of hypotonic fluids, identification of diseases associated with hyponatremia, and cautious correction will decrease the prevalence, incidence, and complications of this common disorder of tonicity.

Four cases histories illustrate the process of defining the cause of hyponatremia: Categorize the patient's volume status; determine kidney function and kidney's response to volume stimulus; measure urine osmolality; consider other factors, including hypouricemia, thyroid status, and adrenal status. Treatment of patients in these four cases is reviewed in the context of CNS tonicity, and the complications encountered during correction of hyponatremic states are described. Specific details are given for administration of parenteral fluids.
