BACKGROUND AND OBJECTIVES: We examined the impact of advance care planning (ACP) self-efficacy and beliefs in explaining skilled nursing facility (SNF) provider judgments about resident need and provider responsibility for initiating ACP conversations. RESEARCH DESIGN AND METHODS: This observational multi-site study of 348 registered nurses, licensed practical nurses, and social workers within 29 SNFs used an anonymous survey in which providers judged vignettes with assigned situational features of a typical SNF resident. Mixed modeling was used to analyze the vignette responses. RESULTS: Providers who had more negative beliefs about ACP were less likely to judge residents in need of ACP and less likely to feel responsible for ensuring ACP took place. Self-efficacy did not have a significant impact on judgments of need, but did significantly increase judgments of responsibility for ensuring ACP conversations. Providers with the highest levels of ACP self-efficacy were most likely to feel responsible for ensuring ACP conversations. In an exploratory analysis, these relationships remained the same whether responding to high or low risk residents (i.e., based on risk of hospitalization, type of diagnosis, functional status, and rate of declining health). DISCUSSION AND IMPLICATIONS: Both negative beliefs about ACP and self-efficacy in one's ability to conduct ACP discussions were associated with professional judgments regarding ACP. The findings illustrate the importance of addressing negative beliefs about ACP and increasing provider ACP self-efficacy through education and policies that empower nurses and social workers.

Objective: To compare health benefits achieved in a transtheoretical model-chronic disease (TM-CD) minimal intervention for obesity vs. augmented usual care (AUC). Research Method and Procedures: This was a 2-year, randomized clinical trial with overweight or obese men and women from 15 primary care sites. AUC (n = 336) included dietary and exercise advice, prescriptions, and three 24-hour dietary recalls every 6 months. TM-CD care (n = 329) included AUC elements plus "stage of change" (SOC) assessments for five target behaviors every other month, mailed SOC and target behavior-matched-workbooks, and monthly telephone calls from a weight-loss advisor. Weight change was the primary outcome. Results: Repeated measures models under the missing at random assumption yielded nonsignificant adjusted differences between. the AUC and TM-CD groups for weight change, waist circumference, energy intake or expenditure, blood pressure, and blood lipids. The pattern of change over time suggested that TM-CD participants were trying harder to impact target behaviors during the first 6 to 12 months of the trial but relapsed afterward. Sixty percent of trial participants maintained their baseline weights for 18 to 24 months. Discussion: A combination of mailed patient materials and monthly telephone calls based on, the transtheoretical model and some elements of chronic disease care is not powerful enough, relative to AUC, to alter target behaviors among overweight primary care patients in an obesogenic environment. AUC may be sufficient to maintain weights among at-risk primary care patients.

Technetium-99m-sestamibi (MIBI) is a new radionuclide for imaging parathyroid tissue; The purpose of this study was to evaluate parathyroid localization using single radiotracer, dual-phase MIBI scintigraphy and to compare the results to ultrasonography. Twenty-one patients with hyperparathyroidism underwent dual-phase scintigraphy using 25 mCi MIBI and high resolution ultrasonography before parathyroidectomy. Scan results were correlated with size, weight, location, and histopathology of excised parathyroid glands, thyroid abnormalities, and cost. Seventeen patients were female, five had secondary or tertiary hyperparathyroidism, and three had a previous parathyroid exploration, Twenty patients (95%) were cured, 14 with a single and 1 with a double adenoma, and 5 of 6 patients with generalized hyperplasia. Therewere no false positive MIBI scans and-one false positive ultrasound study, despite associated thyroid nodules in 29 per cent of patients. The sensitivity of MIBI and ultrasound in the identification of adenomas was 87 per cent versus 57 per cent (P = 0.046), and the rate of detection of hyperplastic glands was 44 per cent versus 24 per cent (P = 0.19), respectively. There was no correlation between scan results and size, weight, or location of adenomatous glands. The cost of dual-phase MIBI was comparable to that of ultrasound. Dual-phase MIBI is more sensitive than ultrasound in the localization of adenomas-and is the preferable modality for preoperative parathyroid localization. Neither MIBI nor ultrasound is effective in localization of hyperplastic glands, underscoring the importance of routine bilateral neck exploration.

Background: Computer-assisted instruction in medicine appears rofacilitare learning, but it has not been compared prospectively to more traditional reaching methods. Purpose: To compare the efficacy of computer-assisted instruction to a more traditional format in medical students' acquisition of clinical acid-base problem-solving skills. Methods: 2nd-year medical students at Northeastern Ohio Universities College of Medicine (N = 82) were randomized into 2 groups. Thirty-seven received computer-assisted instruction in acid-base problem-solving skills, and 45 were enrolled in a teacher-supervised workshop. Preinstruction and postinstruction questionnaires, as well as a 25-question test, were administered to all students. Results: Analysis of the preinstruction questionnaire demonstrated that the students' preferred method of learning was the teacher supervised workshop (50%) followed by reading (39.5%). Fifty-eight percent of the students did not use computers at all. Of the remaining 42%, average computer use was 5.9 days per month. At the end of the course, both groups felt equally prepared for the test. When asked, ''If you had a choice to team the material again, which would you choose: workshop, reading, or computer-assisted instruction?'' 93% of the supervised-workshop group chose the workshop, whereas 53% of the computer-assisted instruction group chose computer-assisted instruction (p < .001). No significant difference between the groups was demonstrated in performance on postinstruction recall questions. We did, however, find a significant difference favoring the workshop group in postinstruction problem-solving skills (p = .04). Conclusions: Computer-assisted instruction can be as effective as a teacher-supervised workshop in transmitting clinical acid-base knowledge skills to medical students. The students, however, do not appear inclined to rely solely on computer programs for learning and seem to benefit from teacher-student interaction in learning problem-solving skills.

Background: Eradication of Helicobacter pylori infection has had an impact on the treatment and recurrence rates of peptic ulcer disease and malignancies such as mucosa-associated lymphoid tissue lymphoma. Treatment options are cumbersome, expensive, and associated with side effects. Methods: Randomized, prospective, open-labeled equivalence trial with a parallel-group design to compare eradication rates of H pylori with a 1-day, 4-drug regimen with a 7-day, 3-drug regimen. A total of 160 patients with dyspepsia and a Glasgow Dyspepsia Severity Score of at least 3 had a urea breath test labeled with carbon 14. Patients who tested positive were randomized to 1 of the 2 study groups. The study was designed to test the therapeutic equivalence of 1-day and 7-day regimens based on the percentage of H pylori eradication in each group at 5 weeks. Results: The 1-day treatment group (n = 80) had a slightly higher eradication percentage (95%) than the 7-day group (90%). The possible inferiority of the 1-day treatment relative to the 7-day treatment, a 15% difference in the number of patients whose infection was not eradicated at 5 weeks, was rejected (P < .001; 90% confidence interval, 2.7%-11%). Both groups demonstrated a mean decrease of 7.5 points in the Glasgow Dyspepsia Severity Score. The 2 groups showed no significant differences in side effects. Patients whose treatment failed (4 in the 1-day treatment group and 7 in the 7-day treatment group) were retreated for 10 days. One patient from the 7-day treatment group still tested positive after the second treatment. Conclusions: The 1-day treatment proved to be statistically similar to the 7-day treatment for the eradication of H pylori in patients with dyspepsia and a positive urea breath test. Further evaluation will be necessary to determine whether the 1-day regimen is adequate for patients with peptic ulcer disease, mucosa-associated lymphoid tissue lymphoma, or gastric adenocarcinoma.

Both consultant and client project; that is, ascribe to the other and to the data aspects they fail to recognize about themselves and about their expectations. This interferes with understanding the research problem and the data. Projection also explains the fascination consultants have with data. Projection of the client onto the consultant and the data hem the consultant into a restricted role that precludes the raising of important questions and a thorough contribution. Mindsets interfere with perception of new information. Recognition of the interference of projections and mindsets allows the consultant to move through the process with greater ease and satisfaction, and, more importantly, to develop the insights needed for a creative resolution to the process.

Various indirect forms of evidence are provided for the accuracy of synthetic estimation of the distribution of severely mentally disabled (SMD) adults across Ohio's mental health board areas. The board areas are public mental health jurisdictions which are allocated state funds to serve the needs of SMD adults. Equitable allocation of funds to these areas depends on accurate estimation of the distribution of SMD adults across the areas. The synthetic estimation method involved two steps. (a) ECA-based prevalences of SMD adults by demographic profiles were obtained, (b) prevalences were applied to a board area's demographic mix to yield the estimated SMD count for that area. The main indirect evidence of accuracy showed that the synthetically-estimated percentages across areas agreed closely with the actual percentages of SMD adults enrolled in federal programs. Statistical characteristics of various percentage estimators indicated that reasonably accurate estimation and allocation is not difficult to attain.

The present study examined the reliability and construct validity in an older adult population of scores on the Behavioral Competence Inventory (BCI), a seven-scale informant measure of adaptive functioning. Study participants included 149 older adults and their informants who participated in a neuropsychological assessment. Results indicated that scores on the seven scales showed adequate internal consistencies and represented seven overlapping but distinct constructs in this population. The BCI's seven scales also discriminated among recommended levels of care. A statistically significant interaction between recommended level of care and current living situation (independent or with family) suggested that the BCI's utility depends on context.

The present study examined whether measurement of hemoglobin-acetaldehyde (HbA1-AcH) using an improved methodology may be useful as a biological marker of alcohol abuse. Red blood cell hemolysates of 182 patients consecutively admitted to the drug and alcohol treatment unit of our institution were analyzed for HbA1-AcH concentration using cation exchange HPLC. Mean HbA1-AcH of those who claimed to drink greater than or equal to 6 drinks/day [mean = 0.055 (% total hemoglobin), SD = 0.051] was significantly higher than the mean of those who drank <6 drinks/day (mean = 0.026, SD = 0.0174). The greatest sum of sensitivity (67%) and specificity (77%) came with a cut-score of 0.030 area% of total hemoglobin. A cut-score of 0.080 produced a 100% specificity, but lowered the sensitivity to 20%. The Pearson product moment correlation (r) between HbA1-AcH and reported drinks per day was r = 0.30 (p < 0.001). There was no significant difference in the association of HbA1-AcH end reported drinking between males and females, and the small difference observed was shown to be entirely associated with differences in hemoglobin levels between the sexes. Cocaine use did not significantly alter the correlation between reported drinking and HbA1-AcH levels. Hemoglobin levels were shown to have a significant correlation with HbA1-AcH independent of drinking. HbA1-AcH was shown to have a better sensitivity and specificity than gamma-glutamyltransferase, ALT, AST, or mean corpuscular volume in this population. The results suggest that HbA1-AcH may be a useful marker to help detect alcohol abuse, especially in populations where other markers have been shown to fail.

An account of the resource
Background. The increasing prevalence of obesity and obesity-related conditions in primary care settings challenges the health care system. Thus. we used available baseline data from an on-going clinical trial to examine the biopsychosocial characteristics of overweight and obese primary care patients. The primary research objective was to ascertain whether theoretically important psychosocial and behavioral variables mediate the empirical relationships between sociodemographic factors and body mass index (BMI). Methods. Overweight or obese primary care patients (n = 665) enrolled in an on-going trial of a cognitive-behavioral obesity intervention provided baseline sociodemographic, psychosocial, nutritional, physical activity, and anthropometric data that were analyzed via multiple regression. Results. Lower educational attainment was associated with a higher BMI after controlling for decisional balance, social support, self-efficacy, energy intake, and energy expenditure (P < 0.05). In contrast, ethnicity was not associated with BMI after controlling for the psychosocial and behavioral variables. Decisional balance and social support variables were associated with BMI and energy intake, but the directions of some of the associations were not consistent with a priori expectations. Conclusions. Overall, we found some evidence of mediation by the psychosocial and behavioral variables of the relationship between ethnicity and BMI, but not for the relationship between education and BMI. In addition, some of the relationships between the psychosocial variables and BMI were opposite of our expectations. (C) 2003 American Health Foundation and Elsevier Science (USA). In elderly individuals, serum creatinine may remain normal as glomerular filtration rate (gfr) declines. Therefore, the estimation of glomerular filtration utilizing mathematical models incorporates age as an important variable. In order to adjust drug dosages and diagnose renal disease earlier in the elderly, a variety of such simplified estimates of gfr have been applied. Unfortunately, no estimator is as accurate as the cumbersome gold standards (e.g. inulin or iothalamate clearance) and the reliability of each may vary with the particular clinical setting. The purpose of this study was to critically evaluate three commonly used estimators of gfr - i.e., creatinine clearance (CC), Cockroft-Gault (CG), and 100 over serum creatinine (100/SC)- comparing them to iothalamate clearance (IC) in a group of healthy ambulatory geriatric subjects (n = 41; ages 65-85). IC declined 1 ml/min per year of age in our sample. CC demonstrated a similar decline, a correlation of 0.83 with IC, and moderate error relative to IC of 17% at the mean (standard error [SE] = 12.3), In contrast, 100/SC correlated only 0.56 with IC, demonstrated a large positive bias (41 ml/min). and showed no age-related decline. An age correction to 100/SC similar to that utilized in the CG formula was clearly necessary. Despite the age and weight correction used in the CG formula, we found the estimates from it to be inaccurate (correlation = 0.5; SE = 23.8). A simpler age-corrected formula (Est. IC = 1/2 [100/SC] + 88 - age) was derived and proved significantly superior to CG in our ambulatory geriatric sample, but still exhibited enough error (SE = 16.4) to question its clinical utility. It appears that serum creatinine based estimates of gfr in the elderly may not provide accurate results.

Objective: We sought to evaluate whether comprehensive postdischarge care management for stroke survivors is superior to organized acute stroke department care with enhanced discharge planning in improving a profile of health and well-being. Methods: This was a randomized trial of a comprehensive postdischarge care management intervention for patients with ischemic stroke and National Institutes of Health Stroke Scale scores greater than or equal to 1 discharged from an acute stroke department. An advanced practice nurse performed an in-home assessment for the intervention group from which an interdisciplinary team developed patient-specific care plans. The advanced practice nurse worked with the primary care physician and patient to implement the plan during the next 6 months. The intervention and usual care groups were compared using a global and closed hypothesis testing strategy. Outcomes fell into 5 domains: (1) neuromotor function, (2) institution time or death, (3) quality of life, (4) management of risk, and (5) stroke knowledge and lifestyle. Results: Treatment effect was near 0 SD for all except the stroke knowledge and lifestyle domain, which showed a significant effect of the intervention (P = .0003). Conclusions: Postdischarge care management was not more effective than organized stroke department care with enhanced discharge planning in most domains in this population. The intervention did, however, fill a postdischarge knowledge gap.

Objective: According to the Transtheoretical Model (TM), tailoring interventions to patients' readiness to change can enhance patient progress and help patients use therapeutic resources more effectively. However, tailoring an intervention depends on accurate assessment of patients' stage of change (SOC). This study describes a multi-item algorithm of SOC (M-SOC) for weight loss-related behaviors that attempts to overcome some of the conceptual and methodological difficulties encountered in previously SOC assessments. Research Methods: Data were collected from participants randomly assigned to the treatment arm of a clinical trial comparing a TM-based, cognitive-behavioral intervention with enhanced usual care for weight loss. Results: The stair-step relationships hypothesized by the TM between assessed SOC and key behavioral measures were found for all algorithms (p < 0.05), indicating that the M-SOC algorithm performs as well as single item algorithms. However, 50% fewer patients were classified in action or maintenance for dietary fat intake and portion control (p < 0.05) by the multiple-item algorithms, providing staging more consistent with the clinical presentation of obese individuals. In addition, logistic regression analyses indicated that the single-item algorithms were not sufficient predictors of these behavioral domains. Discussion: To the extent that treatment is guided by assessed SOC, there is potential for a considerable variability in interventions with different approaches to staging. For weight-loss interventions that target portion control and decreased fat intake, the multi-item SOC algorithms seem to be better guides for matching treatments to SOC.
