Background: In 2019, Navigant Healthcare published research showing that 1 in 5 rural hospitals in Minnesota are at risk of closing as they are not financially sustainable. With 26.7% of Minnesota's population being rural, this is particularly worrisome. A substantial cost to rural hospitals is affording the installation, maintenance, and operation of Computed Tomography (CT) and Magnetic Resonance Imaging (MRI) machines. In light of the serious pressures on rural hospitals, the aim of this paper is to investigate if a disparity exists in MRI and CT machine accessibility among Minnesota's urban and rural county hospitals. Design and methods: Hospitals of Minnesota were contacted and asked how many MRI and CT machines they carried at their facility. This information was compiled in an excel sheet and cross referenced to the county it resided along with the counties: population, rural-urban commuting area (RUCA) classification and land area in square mileage. Results: It was found that the state of Minnesota compared well to the national average in terms of persons and square mileage per MRI and CT machine. When comparing counties of Minnesota by their RUCA classification, a disparity is found in rural counties with regards to square mileage per CT and MRI machine. Conclusions: With distance for service creating a barrier to accessibility, rural county residents would benefit from more inhospital MRI and CT machines. With these findings, it is pertinent further research is conducted to investigate the potential vulnerability of other rural populations with regards to accessibility to radiologic resources.

Gadolinium contrast administration, usually with magnetic resonance imaging, is an important diagnostic modality in the investigation of neurological pathologies. There is little evidence in the literature suggesting repeated exposure to intrathecal gadolinium results in symptoms mimicking post-concussive syndrome (PCS). We studied one patient who received intrathecal gadolinium to investigate a pain pump malfunction and presented with encephalopathic symptoms of confusion and aphasia with imaging consistent with intracranial gadolinium extravasation. The patient was followed up regularly with repeat imaging, reassessment of persistent symptoms, and specialist evaluations; however, symptoms remained refractory and resembled PCS. Our findings indicate a need to further investigate potential associations between intrathecal gadolinium exposure and a clinical presentation consistent with PCS, irrespective of histopathological changes.

Repeated exposure to intravenous gadolinium is thought to have a cumulative effect over time with brain deposits that have been histopathologically confirmed in patients with normal renal function. Acute extravasation of gadolinium contents mimicking acute subarachnoid hemorrhage has also been described elsewhere, especially after intrathecal administration, including the patient in our case report [1,2]. There is, however, no clear consensus yet if these acute “high-dose” or gradually accumulating deposits result in long-term neurological sequelae. Recent studies have demonstrated gadolinium toxicity in preclinical in-vitro studies by pathophysiological mechanisms involving calcium homeostasis [3]. Once within the extracellular space, gadolinium is proposed to have affinity for certain other similar metals with resulting chelation and deposition in the central nervous system [4]. Animal studies have demonstrated increased markers of cellular injury after exposure to higher concentrations of gadolinium [5]. However, causal relationship is difficult to establish between gadolinium administration and neurological sequela [3]. We had the unique opportunity to study one such patient with acute toxic encephalopathy over a period of two years and report subsequent clinical findings.

Calciphylaxis is a deadly disease that is currently diagnosed through a skin biopsy of resultant necrotic lesions despite many cases lacking this characteristic finding. Recently, research has demonstrated the ability of various radiologic techniques to detect calciphylaxis and have promoted their capabilities in earlier diagnosis without tissue invasion. In this case, an obese 55-year-old female with end stage renal disease, and a long history of dialysis, complained of weeks of persistent abdominal pain that was accompanied by a mottled, lacey, net-like rash that resembled livedo reticularis. Computed tomography of the abdomen revealed extensive arterial calcification and subcutaneous calcium deposition. These radiologic findings, coupled to a high clinical suspicion, prompted treatment for suspected calciphylaxis. Remarkably, after 1 week the patient reported substantial improvement. Hopefully, this publication in conjunction with previous and future research will raise awareness on the role Radiologists can play in expediting the diagnostic process for a lethal disease, especially when a tissue biopsy is not a feasible option.

Screening for breast cancer reduces breast cancer-related mortality and earlier detection facilitates less aggressive treatment. Unfortunately, current screening modalities are imperfect, suffering from limited sensitivity and high false-positive rates. Novel techniques in the field of breast imaging may soon play a role in breast cancer screening: digital breast tomosynthesis, contrast material-enhanced spectral mammography, US (automated three-dimensional breast US, transmission tomography, elastography, optoacoustic imaging), MRI (abbreviated and ultrafast, diffusion-weighted imaging), and molecular breast imaging. Artificial intelligence and radiomics have the potential to further improve screening strategies. Furthermore, nonimaging-based screening tests such as liquid biopsy and breathing tests may transform the screening landscape. © RSNA, 2020 Online supplemental material is available for this article.

This multidisciplinary update of the Society of Radiologists in Ultrasound consensus statement on liver elastography incorporates the large volume of new information available in the literature since the initial publication. The recommended procedure for acquiring stiffness measurements is reviewed. There has been substantial improvement in the acoustic radiation force impulse (ARFI) technology-most notably the addition of a quality assessment of the shear wave propagation. Due to the efforts of the Quantitative Imaging Biomarkers Alliance, or QIBA, the variability of liver stiffness measurements between systems had decreased. There are now effective treatments for hepatitis B and hepatitis C, and follow-up after effective treatment should be based on the use of the delta change of the value obtained at viral eradication or suppression. Because the detection of compensated advanced chronic liver disease (cACLD) is very important, the new guidelines are made based on the probability of cACLD for given stiffness values. The panel recommends a vendor-neutral rule of four for interpretation for ARFI techniques. This new method simplifies interpretation of liver stiffness results and is more clinically relevant.

An account of the resource
The composition of cartilage is predictive of its in vivo performance. Therefore, the ability to assess its primary macromolecular components, proteoglycan (PG) and collagen, is of great importance. In the current study, we hypothesized that PG content and distribution in tissue engineered cartilage could be determined using Fourier-transform infrared imaging spectroscopy (FT-IRIS). The cartilage was grown from chondrocytes within a hollow fiber bioreactor (HFBR) system previously used extensively to study cartilage development. FT-IRIS analysis showed a gradient of PG content, with the highest content in the center near the nutritive fibers and the lowest near the interior surface of the HFBR. Further, we found significantly greater PG content in the region near culture medium inflow (45.0%) as compared to the outflow region (24.7%) (p < 0.001). This difference paralleled the biochemically determined glycosaminoglycan difference of 42.6% versus 27.8%. In addition, FT-IRIS-determined PG content at specific positions within the tissue sections correlated with histologically determined PG content (R=50.73, p=50.007). In summary, FT-IRIS determination of PG correlates with histological determination of PG and yields quantitatively similar results to biochemical determination of glycosaminoglycan in developing cartilage. Positron emission tomography (PET), with the use of F-18 fluorodeoxyglucose (FDG), has been shown to be useful in the evaluation of many tumors as a result of its high sensitivity and specificity. There are instances, however, in which false-positive FDG uptake can occur with inflammatory lesions or granulomatous diseases. In fact, false-positive mediastinal uptake in thoracic sarcoidosis has been well documented. We present a case of false-positive FDG uptake in the liver resulting from hepatic sarcoidosis. A 42-year-old, asymptomatic woman underwent a PET scan after a solitary pulmonary nodule was detected on a chest CT. The nodule in the left lower lobe on CT revealed no metabolic activity, but 2 lesions were found in the liver with SUVs in the range of 2.5 to 3.2. A subsequent MRI was not able to exclude malignancy. The liver lesions were surgically removed and found to be sarcoidosis.

Objectives-The purpose of this study was to determine whether the elasticity imaging/B-mode ratio on strain elastography can predict breast cancer tumor grades. Methods-A retrospective review of patients with breast lesions who underwent strain elastography and had a diagnosis of breast cancer by image-guided or surgical biopsy was performed. The axis of the maximum elastographic dimension was compared to the B-mode dimension to form an elasticity imaging/B-mode ratio. Lesions were categorized according to their pathologic type, including atypical ductal hyperplasia (ADH), mucinous or colloid cancer, ductal carcinoma in situ (DCIS), grade I invasive ductal carcinoma (MC), grade II IDC, grade III DC, invasive lobular carcinoma (ILC), and lymphoma. The mean elasticity imaging/B-mode ratio of each tumor type was calculated. The elasticity imaging/B-mode ratio of the tumor was compared to the tumor type by Kruskal-Wallis and Tukey-Kramer tests (lymphoma and ADH excluded because of small numbers). Results-Tumor grades included lymphoma (n = 3),.ADH (n = 2), mucinous cancer (n = 11), DCIS (n = 19), DC (grades I III; n = 200), and ILC (n = 31). The mean elasticity imaging/B-mode ratio varied with increasing tumor grade. Tumor grades could not have been selected at random from one population (P < .0001, chi(2) test). Invasive lobular carcinoma and grade III DC were statistically different from mucinous or colloid cancer, DCIS, and grade land II IDC. Conclusions-The elasticity imaging/B-mode ratio on strain elastography is related to the tumor grade.

An account of the resource
Purpose: To prospectively evaluate the performance of real-time ultrasonographic (US) shear-wave elastography (SWE) in the diagnosis of peripheral zone prostate cancer in patients with high and/or increasing prostate-specific antigen levels and/or abnormal digital rectal examination results. Materials and Methods: After signing an informed consent form, men referred for transrectal prostate biopsy were enrolled in this prospective HIPAA-compliant two-center study, which was conducted with institutional review board approval. Transrectal US SWE of the prostate was performed after a conventional transrectal US examination and immediately before US-guided 12-core sextant biopsy. For each sextant, the maximum SWE value was measured and matched to the pathologic results of that sextant biopsy. The diagnostic performance of SWE was assessed at both patient and sextant levels. The elasticity value maximizing the Youden index was used to derive sensitivity, specificity, positive predictive value (PPV), and negative predictive value (NPV). Results: The elasticity values were matched to pathologic results for a total of 1040 peripheral zone sextants in 184 men. One hundred twenty-nine positive biopsy findings (size,. 3 mm; Gleason score,. 6) were identified in 68 patients. The sextant-level sensitivity, specificity, PPV, NPV, and area under the receiver operating characteristic curve for SWE with a cutoff of 35 kPa for differentiating benign from malignant lesions were 96% (95% confidence interval [CI]: 95%, 97%), 85% (95% CI: 83%, 87%), 48% (95% CI: 46%, 50%), 99% (95% CI: 98%, 100%), and 95% (95% CI: 93%, 97%), respectively. Conclusion: Use of a 35-kPa threshold at SWE may provide additional information for the detection and biopsy guidance of prostate cancer, enabling a substantial reduction in the number of biopsies while ensuring that few peripheral zone adenocarcinomas are missed.
