The purpose of this study was twofold: 1) to examine photoperiod-dependent and steroid-dependent adjustments in hypothalamic GnRH and catecholamine content in male deer mice, Peromyscus maniculatus, and 2) to evaluate whether the commonly used measure of GnRH accumulation serves as an accurate indicator of altered reproductive function in short-day-housed rodents, Deer mice provide a unique animal model in which to evaluate the effect of short day length on the neural substrates directing reproductive function, since individual deer mice respond to short photoperiod with a range of neuroendocrine, endocrine, metabolic, and thermoregulatory adjustments. These adjustments, in part, result in two distinct reproductive phenotypes; reproductively competent ''nonresponsive'' mice, and gonad-regressed ''responsive'' mice. Reproductively mature males were maintained on either long (16L:8D) or short (8L:16D) photoperiod for 8 wk. Thereafter, mice were phenotyped and either remained intact, castrated, or castrated and provided with an s.c. testosterone-filled silicons elastomer capsule releasing long-day or twice long-day levels of testosterone; mice were then housed for an additional 4 wk. At autopsy, plasma levels of LH and testosterone, hypothalamic GnRH content, and hypothalamic dopamine and norepinephrine concentrations were determined. Short photoperiod caused a similar increase in GnRH content in both short-day phenotypes, and this increase was primarily localized to the medial basal hypothalamus (MBH). Castration decreased GnRH content within the MBH of mice housed on both a long and short photoperiod. However, castration also reduced GnRH content within the medial preoptic area (MPOA) only among the short-day phenotypes. Steroid replacement at levels similar to those of intact long-day males restored GnRH content in all brain regions to levels of intact mice. On the other hand, steroid replacement at twice long-day levels restored GnRH content among long-day mice but reduced GnRH content among both short-day reproductive response groups, specifically within the MBH and MPOA. There were no dramatic effects of photoperiod or steroid treatment on catecholamine concentrations. But our results do suggest that the lateral preoptic area (LPOA) and MBH may be important sites of steroid feedback among mice exposed to short photoperiods and that the LPOA and diagonal band of Broca-septal regions may specifically be associated with catecholamine-directed photoperiod-induced adjustments of reproductive responsive mice. Taken together, these data show that GnRH accumulation is not the proximate signal directing seasonal changes in pituitary-gonadal function and cast doubt on the use of this factor as an accurate indicator of altered reproductive function.

The progestin-induced withdrawal bleeding test has often been used in patients with amenorrhea to assess endogenous estrogen (E) production. The endometrial thickness measured by vaginal ultrasonography with a 5 MHz transducer is also dependent on E stimulation of the endometrium. In this study, 70 consecutive patients were evaluated with both the progestin-induced withdrawal bleeding test and a measurement of the endometrial thickness by vaginosonography. An endometrial thickness of 1.5 mm or less was selected to predict absence of bleeding after a progesterone (P) challenge test. This resulted in a sensitivity of 94% with a 95% confidence interval (CI) of 0.70 to 1.00 and a specificity of 93% with a 95% CI of 0.82 and 0.98. The positive and negative predictive values were 79% and 98%, respectively. This study shows that the endometrial thickness measured by vaginosonography can predict the results of the P challenge test in patients with amenorrhea.

Objective: To determine the efficacy of treatment of significant hirsutism with a GnRH agonist (GnRH-a) and estrogen and progestin replacement therapy. Design: Clinical series. Setting: Ambulatory gynecology clinic in a community hospital. Patients: Ten women with significant hirsutism caused by polycystic ovarian disease. Interventions: The patients were treated with leuprolide acetate 20 mu g/kg per day in combination with E(2) (2 mg) and medroxyprogesterone acetate (5 mg) for 6 months. Main Outcome Measure: Hirsutism scores and hair growth rates determined before and upon completion of treatment protocol. Results: Hirsutism scores and hair growth rates significantly decreased by 23% and 26%, respectively. The duration of hirsutism was the only significant covariate for hirsutism scores and hair growth rates. Only two patients had minimal, irregular bleeding that was corrected by increasing the estrogen dose. Conclusion: The combination of a GnRH-a and estrogen replacement therapy was an effective and well-tolerated treatment in a small group of women with significant hirsutism caused by PCOD.

Background: VEGF-regulated genes in the cervices of pregnant and non-pregnant rodents (rats and mice) were delineated by DNA microarray and Real Time PCR, after locally altering levels of or action of VEGF using VEGF agents, namely siRNA, VEGF receptor antagonist and mouse VEGF recombinant protein. Methods: Tissues were analyzed by genome-wide DNA microarray analysis, Real-time and gel-based PCR, and SEM, to decipher VEGF function during cervical remodeling. Data were analyzed by EASE score (microarray) and ANOVA (Real Time PCR) followed by Scheffe's F-test for multiple comparisons. Results: Of the 30,000 genes analyzed, about 4,200 genes were altered in expression by VEGF, i.e., expression of about 2,400 and 1,700 genes were down- and up-regulated, respectively. Based on EASE score, i.e., grouping of genes according to their biological process, cell component and molecular functions, a number of vascular- and non-vascular-related processes were found to be regulated by VEGF in the cervix, including immune response (including inflammatory), cell proliferation, protein kinase activity, and cell adhesion molecule activity. Of interest, mRNA levels of a select group of genes, known to or with potential to influence cervical remodeling were altered. For example, real time PCR analysis showed that levels of VCAM-1, a key molecule in leukocyte recruitment, endothelial adhesion, and subsequent trans-endothelial migration, were elevated about 10 folds by VEGF. Further, VEGF agents also altered mRNA levels of decorin, which is involved in cervical collagen fibrillogenesis, and expression of eNO, PLC and PKC mRNA, critical downstream mediators of VEGF. Of note, we show that VEGF may regulate cervical epithelial proliferation, as revealed by SEM. Conclusion: These data are important in that they shed new insights in VEGF's possible roles and mechanisms in cervical events near-term, including cervical remodeling.
