OBJECTIVE: The main purpose of this article is to evaluate whether identification and treatment of women with mild gestational diabetes mellitus (GDM) during pregnancy affects subsequent maternal body mass index (BMI), anthropometry, metabolic syndrome, and risk of diabetes. STUDY DESIGN: This is a follow-up study of women who participated in a randomized controlled treatment trial for mild GDM. Women were enrolled between 5 and 10 years after their index pregnancy. Participants underwent blood pressure, height, weight, and anthropometric measurements by trained nursing personnel using a standardized approach. A nurse-assisted questionnaire regarding screening and treatment of diabetes or hypercholesterolemia, diet, and physical activity was completed. Laboratory evaluation included fasting serum glucose, fasting insulin, oral glucose tolerance test, and a lipid panel. Subsequent diabetes, metabolic syndrome, obesity, and adiposity in those diagnosed with mild GDM and randomized to nutritional counseling and medical therapy (treated) were compared with those who underwent routine pregnancy management (untreated). Multivariable analyses were performed adjusting for race/ethnicity and years between randomization and follow-up visit. RESULTS: Four-hundred fifty-seven women with mild GDM during the index pregnancy were included in this analysis (243 treated; 214 untreated) and evaluated at a median 7 years after their index pregnancy. Baseline and follow-up characteristics were similar between treatment groups. Frequency of diabetes (9.2 vs. 8.5%, p =0.80), metabolic syndrome (32.2 vs. 34.3%, p =0.63), as well as adjusted mean values of homeostasis model assessment for insulin resistance (2.5 vs. 2.3, p =0.11) and BMI (29.4 vs. 29.1 kg/m(2), p =0.67) were also not different. CONCLUSION: Identification and treatment of women with mild GDM during pregnancy had no discernible impact on subsequent diabetes, metabolic syndrome, or obesity 7 years after delivery.

BACKGROUND: In recent years, the use of adrenocorticotropic hormone (ACTH) therapy for treatment of proteinuria due to nephrotic syndrome (NS) has been heavily explored. ACTH therapy, which comes in the natural (H. P. Acthar Gel) or synthetic (tetracosactide) form, has resulted in remission in patients with immunosuppressive and steroid-resistant NS. However, the exact efficacy of ACTH therapy in the NS etiologies, such as membranous nephropathy (MN), focal segmental glomerulosclerosis (FSGS), minimal change disease (MCD), lupus nephritis (LN), IgA nephropathy (IgAN), and membranoproliferative glomerulonephritis (MPGN), has not been determined. OBJECTIVE: This systematic review analyzed the published literature on ACTH therapy in various NS etiologies to determine its efficacy. METHODS: A comprehensive search of MEDLINE, EMBASE, and Cochrane databases was conducted for articles through June 2019. An additional search was performed on clinicaltrials.gov to search for additional trials and cross reference the results of our database search. The literature which studied synthetic or natural ACTH treatment in patients with known etiologies of NS was included. Studies were excluded when they consisted of a single case report or did not analyze the lone effect of ACTH in NS. RESULTS: The initial search yielded a total of 411 papers, and 22 papers were included. In 214 MN patients, there was an overall remission of 40% (85/214) and an overall remission of 43% (42/98) in FSGS patients. In other etiologies, there were overall remissions of 78% (11/14), 31% (5/16), 40% (16/40), and 62% (8/13) in MCD, LN, IgAN, and MPGN patients, respectively. CONCLUSION: ACTH showed benefits in proteinuria reduction across all etiologies of NS. However, more randomized controlled studies with larger population sets and longer follow-ups are imperative to establish causal benefits. New studies into its efficacy in children are also necessary.

Bowhead whales are some of the largest animals that occupy the Arctic Circle. Despite the challenges of living and giving birth in icy waters, having huge blubber stores, eating a fat-rich diet, and undergoing arduous migrations, bowheads achieved the longest known life span of mammals of 268 years. Their longevity is extended by fixed mutations that prevent DNA damage and cancer and through evolutionary modifications to their metabolism that compensate for an oxygen-poor environment. Recently, the bowhead genome and transcriptome libraries were made publicly available for study. Analyses suggest that their life span has been extended by evolutionary changes that result in the upregulation of DNA repair pathways. Molecular biologists are now undertaking laboratory experiments with whale samples that are informed by the bowhead genome and transcriptome to tackle questions that were inaccessible using classical model organisms such as rodents or fish. Biomedical researchers are also applying insights gained from research on bowheads into investigations of potential therapies for aging, senescence, and cancer. Moreover, researchers are using these results to inform our understanding of the evolutionary history of these traits. This chapter reviews the molecular basis for bowhead whale longevity and survival in their unique habitat and offers insights for further exploration into the molecular mechanisms that shape the extraordinary lives of these animals.

BACKGROUND: Interventional therapies aiming at excluding the left atrial appendage (LAA) from systemic circulation have been established as a valid alternative to oral anticoagulation in patients at high thromboembolic risk. However, their efficacy on stroke prophylaxis may be compromised owing to incomplete LAA closure. Additionally, the need for an alternative thromboembolic prevention may remain unmet in patients with contraindications to oral anticoagulation whose appendage anatomy is unsuitable for some conventional devices commercially available. We aimed at evaluating the feasibility of LAA closure with the novel Gore Cardioform Septal Occluder in patients with incomplete appendage ligation or anatomic features which do not meet the manufacturer's requirements for Watchman deployment. METHODS: Twenty-one consecutive patients (mean age: 72±6 years; 85.7% males; CHA(2)DS(2)-VASc: 4.5±1.4; HAS-BLED: 3.6±1.0) were included. Transesophageal echocardiography was performed within 2 months to assess for residual LAA patency. RESULTS: Fourteen patients had incomplete LAA closure following surgical (n=6) or Lariat ligation (n=8). In 7 patients with an appendage anatomy unsuitable for Watchman deployment, the mean maximal landing zone size and LAA depth were 14.4±1.3and 18.6±2.8 mm. Successful Cardioform Septal Occluder deployment was achieved in all patients. No peri-procedural complications were documented.Procedure and fluoroscopy times were 46±13 and 14±5 minutes. Follow-up transesophageal echocardiography after 58±9 days revealed complete LAA closure in all patients. CONCLUSIONS: Transcatheter LAA closure via a Cardioform Septal Occluder device might be a valid alternative in patients with residual leaks following failed appendage ligation or whose LAA anatomy does not meet the minimal anatomic criteria to accommodate a Watchman device.

Swallowing in mammals requires the precise coordination of multiple oropharyngeal structures, including the palatopharyngeal arch. During a typical swallow, the activity of the palatopharyngeus muscle produces pharyngeal shortening to assist in producing pressure required to swallow and may initiate epiglottal flipping to protect the airway. Most research on the role of the palatopharyngeal arch in swallowing has used pharyngeal manometry, which measures the relative pressures in the oropharynx, but does not quantify the movements of the structures involved in swallowing. In this study, we assessed palatopharyngeal arch and soft palate function by comparing their movements in a healthy population to a pathophysiological population longitudinally through infancy (term versus preterm pigs). In doing so, we test the impact of birth status, postnatal maturation, and their interaction on swallowing. We tracked the three-dimensional (3D) movements of radiopaque beads implanted into relevant anatomical structures and recorded feeding via biplanar high-speed videofluoroscopy. We then calculated the total 3D excursion of the arch and soft palate, the orientation of arch movement, and the timing of maximal arch constriction during each swallow. Soft palate excursion was greater in term infants at both 7 and 17 days postnatal, whereas arch excursion was largely unaffected by birth status. Maximal arch constriction occurred much earlier in preterm pigs relative to term pigs, a result that was consistent across age. There was no effect of postnatal age on arch or soft palate excursion. Preterm and term infants differed in their orientation of arch movement, which most likely reflects both differences in anatomy and differences in feeding posture. Our results suggest that the timing and coordination of oropharyngeal movements may be more important to feeding performance than the movements of isolated structures, and that differences in the neural control of swallowing and its maturation in preterm and term infants may explain preterm swallowing deficits.

OBJECTIVE: To evaluate characteristics associated with adverse outcomes in low-risk nulliparous women randomized to elective labor induction at 39 weeks of gestation or expectant management. METHODS: We conducted a secondary analysis of women randomized during the 38th week to induction at 39 weeks of gestation or expectant management. Deliveries before 39 weeks of gestation and those not adherent to study protocol or with fetal anomalies were excluded. A composite of adverse outcomes (perinatal death or severe neonatal complications), third- or fourth-degree lacerations, and postpartum hemorrhage were evaluated. Log binomial regression models estimated relative risks and 95% CIs for associations of outcomes with patient characteristics including randomly assigned treatment group. Interactions between patient characteristics and treatment group were tested. RESULTS: Of 6,096 women with outcome data, 5,007 (82.1%) met criteria for inclusion in this analysis. Frequency of the perinatal composite was 252 (5.0%), 166 (3.3%) for third- or fourth-degree perineal laceration, and 237 (4.7%) for postpartum hemorrhage. In multivariable analysis, intended labor induction at 39 weeks of gestation was associated with a reduced perinatal composite outcome (4.1% vs 6.0%; adjusted relative risk [aRR] 0.71; 95% CI 0.55-0.90), whereas increasing body mass index (BMI) was associated with an increased perinatal composite outcome (aRR 1.04/unit increase; 95% CI 1.02-1.05). Decreased risk of third- or fourth-degree perineal laceration was observed with increasing BMI (aRR 0.96/unit increase; 95% CI 0.93-0.98) and in Black women compared with White women (1.2% vs 3.9%; aRR 0.34; 95% CI 0.19-0.60). Increased risk of postpartum hemorrhage was observed in Hispanic women compared with White women (6.3% vs 4.0%; aRR 1.64; 95% CI 1.18-2.29). Patient characteristics associated with adverse outcomes were similar between treatment groups (P for interaction >.05). CONCLUSION: Compared with expectant management, intended induction at 39 weeks of gestation was associated with reduced risk of adverse perinatal outcome. Patient characteristics associated with adverse outcomes were few and similar between groups. CLINICAL TRIAL REGISTRATION: ClinicalTrials.gov, NCT01990612.

In the late 1980s, annual carbon isotope cycles in the baleen plates of bowhead whales formed the basis of the first effective means of estimating bowhead age. However, the baleen aging method could only be used for aging subadult whales less than about 20 years old, due to wearing of the plates. Since then, seven additional methods of estimating the age of bowheads have been developed. Each age estimation method has strengths and weaknesses depending on the life stage of the whale. No single method is fully applicable through the entire 200-year lifespan of a bowhead, although a combination of techniques can be quite effective and precise.

Bering–Chukchi–Beaufort Sea bowhead whales are born at about 4.2m in length and 1000kg, in lead systems along the NW Alaskan coast. Bowheads in the other northern stocks are also born in ice-covered seas. Maximum body lengths (standard measure) can reach 19m. Regarding coloration: the skin is gray on neonates and young calves, and black on yearlings and adults. The size and shape of the chin patch is variable and does not change through life. After adulthood age, the specific areas of the skin lose pigment and turn white around the eye, the peduncle, genital groove, and base of the pectoral limb. Growth is rapid from birth to age 1, followed by a 3- to 4-year growth hiatus. Following the hiatus, the increase in body length is slow to sexual maturity at ~25years and ~13.5m in length. The Iñupiat recognize several growth forms associated with age; these include aġvaaq (neonate), ingutuq (yearling), qairaliq (postyearling), along with several names for large adults. Allometry of the head is marked and likely associated with extreme development of the baleen rack. The head is very large in relation to body—perhaps the largest in Mammalia. The blubber is among the thickest of any cetacean (to ~38cm). Body mass is similar to right whales of similar length; large bowheads can reach 100 metric tons or larger. Longevity can reach 200 or more years. The age of physical maturity is unclear, and adults may increase in length through life. Their ability to navigate ice-covered seas and break through ice to breathe is singular among baleen whales.

Study design is critical to ensure that research questions are answered in an appropriate and rational manner for all aspects of health, but particularly in emergency care. Appropriate study design selection is one of the most critical decisions to make at the earliest stage of a research project; once this is clear, much of the methodology and sample size estimations should be straightforward. Selection of an appropriate study design is fundamental to good research and deserves careful consideration at the outset of any research project. The classic gold standard for study design is the double-blind randomised clinical trial, but it is often not possible to achieve this ideal in the busy clinical emergency environment or with the resources available. Descriptive studies are common in emergency care; they include retrospective clinical records reviews, prospective cohort studies and case-control studies. Case reports and surveys can be a useful introduction to research for novice researchers. When sufficient empirical evidence on a topic exists, results of similar studies can be combined in systematic reviews and/or meta-analyses to pool the results from multiple studies to determine stronger evidence for or against an intervention or treatment, but these techniques require specialist expertise and statistical skills.

The skull of the bowhead whale is composed of all the bones commonly found in mammals, although the shape of these bones is far from ordinary. Features related to feeding dominate the skull: there are no teeth, and the rostrum is long and curved, resembling a bow. The incisive and maxilla form a high arch that forms the origin of the baleen rack, and the mandibles are straight in lateral view, but deeply curved in the horizontal plane to make room for the tongue and baleen. The blowhole is located near the apex of the skull, and the boney portion of the nasal passage opens caudally into the nasopharynx ventral to the braincase. As in other baleen whales, the occipital projects rostrally, covering a portion of the parietals and frontals.

Solid organ transplantation in children and adolescents provides many benefits through improving critical organ function, including better growth, development, cardiovascular status, and quality of life. Unfortunately, bone status may be adversely affected even when overall status is improving, due to issues with pre-existing bone disease as well as medications and nutritional challenges inherent post-transplantation. For all children and adolescents, bone status entering adulthood is a critical determinant of bone health through adulthood. The overall health and bone status of transplant recipients benefits from attention to regular physical activity, good nutrition, adequate calcium, phosphorous, magnesium and vitamin D intake and avoidance/minimization of soda, extra sodium, and obesity. Many immunosuppressive agents, especially glucocorticoids, can adversely affect bone function and development. Minimizing exposure to "bone-toxic" medications is an important part of promoting bone health in children post-transplantation. Existing guidelines detail how regular monitoring of bone status and biochemical markers can help detect bone abnormalities early and facilitate valuable bone-directed interventions. Attention to calcium and vitamin D supplementation, as well as tapering and withdrawing glucocorticoids as early as possible after transplant, can provide best bone outcomes for these children. Dual-energy X-ray absorptiometry can be useful to detect abnormal bone mass and fracture risk in this population and newer bone assessment methods are being evaluated in children at risk for poor bone outcomes. Newer bone therapies being explored in adults with transplants, particularly bisphosphonates and the RANKL inhibitor denosumab, may offer promise for children with low bone mass post-transplantation.

An account of the resource
The morbidity and mortality of adult and pediatric chronic kidney disease (CKD) and end-stage renal disease (ESRD) populations are mainly driven by cardiovascular disease (CVD). Improving CVD outcomes focuses on risk assessment of factors including diastolic blood pressure (DBP), systolic blood pressure (SBP), left ventricular mass index (LVMI), pulse pressure (PP), and pulse pressure index (PPi), which is calculated as PP/SBP. These markers are also proven predictors of CKD progression; however, their role in children has not been established. This study aims to evaluate the relationship between PP, PPi, ambulatory arterial stiffness index (AASI), and proteinuria with kidney function in pediatric CKD patients; it is a retrospective analysis of 620 patients (1-16 years) from the NIDDK Chronic Kidney Disease in Children (CKiD) registry. The authors analyzed data for three separate cohorts: an overall CKD as well as immunological versus non-immunological cause for CKD groups. An inverse relationship was found between SBP, DBP, and PP with iGFR and LVMI in the overall CKD group. Our immunological CKD subgroup showed significantly higher serum creatinine, SBP, DBP, and PP values with significantly lower serum albumin levels compared to the non-immunological group. There were no significant differences with iohexol-based glomerular filtration rate (iGFR), LVMI, PPi, or high-sensitivity C-reactive protein (hs-CRP) between the two groups. A subgroup analysis demonstrated that SBP, DBP, and PP all correlated significantly with LVMI in the immunological CKD patients but not the non-immunological subgroup. Additionally, AASI data in the overall CKD population were significantly correlated with PP, PPi, and DBP. This study is one of the first to correlate noninvasive measurements of vascular compliance including PP, PPi, and AASI with iGFR and LVMI in a pediatric CKD cohort. Improving our understanding of surrogate markers for early CVD is integral to improving the care of pediatric CKD population as these patients have yet to develop the hard end points of ESRD, heart failure, myocardial infarction, or stroke.

An account of the resource
An understanding of adult day service centers' (ADC) impacts on clients' health and well-being has been hampered by a lack of large-scale data. Standardizing data collection is critical to strengthening ADC programs, demonstrating their effectiveness, and enabling them to leverage additional funding streams beyond Medicaid. We distributed an electronic survey on current data collection efforts to ADCs nationally to determine categories of data ADCs are collecting related to clients' health. In our sample (N = 248), only 32% of ADCs collected patient-level data for research and analysis-most commonly on activities of daily living, cognition, nutrition, and caregiver strain. However, validated assessment tools were used in less than 50% of the cases. ADCs are willing to collect data: More than 70% reported a willingness to participate in future studies. National studies piloting data collection protocols with uniform outcome measures are needed to advance the understanding of ADCs' capabilities and impacts.

The specialty of Emergency Medicine continues to expand and mature worldwide. As a relatively new specialty, the body of research that underpins patient management in the emergency department (ED) setting needs to be expanded for optimum patient care. Research in the ED, however, is complicated by a number of issues including limited time and resources, urgency for some therapeutic investigations and interventions, and difficulties in obtaining truly informed patient consent. Notwithstanding these issues, many of the fundamental principles of medical research apply equally to ED research. In all medical disciplines, data needs to be collected, collated and stored for analysis and a data spreadsheet is employed for this purpose. Like other aspects of clinical research, the use of the data spreadsheet needs to be exacting and appropriate. This research primer explores the choice of available spreadsheets and a range of principles for their best-practice use. It is deliberately, not an exhaustive review of the subject. However, we aim to explore basic principles and some of the most accessible and widely used data spreadsheets.

The commercial hunt of the four stocks of bowhead whales by the nations of Europe and North America commenced in 1540 and came to an effective end at the start of World War I. At that time, all four stocks had been driven to near extinction. Whalers from many nations were involved, but the most important ones were those from Basque Country, the Netherlands, Germany, Great Britain, and the United States. The Spitsbergen stock was the first to be depleted, followed by the West Greenland–East Canada stock (including whaling efforts in Hudson Bay and Strait of Belle Isle) and, finally, the Okhotsk and Bering–Chukchi–Beaufort stocks. Competition with other products (e.g., gaslight and petroleum), political factors (e.g., US Civil War), and changes in demands (e.g., women’s fashion) caused a waxing and waning of the whaling industry, but the decrease in yield eventually stopped the hunt. Native peoples across the circumpolar Arctic suffered greatly even though commercial whalers sometimes had good relations with native people they encountered. Despite any possible good intentions, the end result was, on balance, negative and led to major changes in the indigenous cultures it affected.

We summarize its relations to humans, including whaling by indigenous and European whalers, conservation efforts, and modern challenges with increased shipping, fishing, and climate change. The sensory biology of bowhead whales reflects features both related to their origin from land mammals and adaptations to their current environment, polar seas. There is anatomical and genomic evidence that bowheads have a sense of smell. Their sense of taste may be limited to detecting salty flavorants and there is no vomeronasal organ. They lack binocular vision and color vision, and it is likely that they can perceive the magnetic field. Similar to other mysticetes, their organ of hearing is adjusted to low frequencies. Their organ of balance is much smaller than could be expected when compared to land mammals, and the functional reason for this is unclear. Sensory hairs on the face may detect water current, and, when exposed to air, wind direction.

The prenatal development of bowhead whales is poorly known, and no complete ontogenetic series exist. However, the available embryos and fetuses elucidate aspects of the development of the species, as well as that of all of mysticetes in general, and demonstrate the role of heterochrony in development. For instance, hair on the face of bowhead whale embryos develops at the same ontogenetic stage as it does in terrestrial mammals, even though body hair never develops. The absence of teeth characterizes all modern baleen whales, but bowhead whale fetuses initiate teeth around the same time as land mammals, to then resorb them in the fetal period. Baleen forms after that, late in the fetal period.

An account of the resource
The postcranial skeleton of bowhead whales was described in detail more than 100 years ago. The musculature of bowheads has not been studied in detail but matches that of other mysticetes in general features. In this chapter, we focus on some aspects of the skeleton that differs from that of other cetaceans. First, the skeleton of bowheads changes with ontogenetic age. Some of these changes are gross-morphologically, while others are biochemical and histological. Several are related to the life history of the species. Our discussion focuses on the changes in the morphology of the cervical vertebral column, the ontogenetic variation in carpals, and the variation in the hind limb. Individuals with external hind limbs occur infrequently, but there is a great variation in the size of ox coxae, femur, and tibia.

Over the past four decades, there have been progressive changes in the epidemiology of traumatic spinal cord injury (tSCI). We assessed trends in demographic and injury-related variables in traumatic cervical spinal cord injury (tCSCI) patients over an 18-year period at a single Level I trauma center. We included all magnetic resonance imaging-confirmed tCSCI patients ≥15 years of age for years 2001-2018. Among 1420 patients, 78.3% were male with a mean age 51.5 years. Etiology included falls (46.9%), motor vehicle collisions (MVCs; 34.2%), and sports injuries (10.9%). Median American Spinal Injury Association (ASIA) Motor Score (AMS) was 44, complete tCSCI was noted in 29.6% of patients, fracture dislocations were noted in 44.7%, and median intramedullary lesion length (IMLL) was 30.8 mm (complete injuries 56.3 mm and incomplete injuries 27.4 mm). Over the study period, mean age and proportion of falls increased (p < 0.001) whereas proportion attributable to MVCs and sports injuries decreased (p < 0.001). Incomplete injuries, AMS, and the proportion of patients with no fracture dislocations increased whereas complete injuries decreased significantly. IMLL declined (p = 0.17) and proportion with hematomyelia did not change significantly. In adjusted regression models, increase in age and decreases in prevalence of MVC mechanism and complete injuries over time remained statistically significant. Changes in demographic and injury-related characteristics of tCSCI patients over time may help explain the observed improvement in outcomes. Further, improved clinical outcomes and drop in IMLL may reflect improvements in initial risk assessment and pre-hospital management, advances in healthcare delivery, and preventive measures including public education.

Objective: To determine the effect of electrocardiogram (ECG) screening on the prevention of sudden cardiac arrest and death in young athletes and military members.
Data sources: MEDLINE, Embase, CENTRAL, Web of Science, BIOSIS, Scopus, SPORTDiscus, PEDro, and ClinicalTrials.gov were searched from inception to dates between February 21 and July 29, 2019.
Study selection: Randomized and nonrandomized controlled trials in which preparticipation examination including ECG was the primary intervention used to screen athletes or military members aged ≤40 years. Acceptable control groups were those receiving no screening, usual care, or preparticipation examination without ECG. Three published studies and 1 conference abstract were identified for inclusion.
Data extraction: In all 4 studies, risk of bias was assessed using the Cochrane risk-of-bias tool and was found to be generally high. Two studies had data extracted for random effects meta-analysis, and the remaining study and conference abstract were included in the narrative review. The overall quality of evidence was assessed using the Grading of Recommendations, Assessment, Development and Evaluation approach.
Data synthesis: We included 4 nonrandomized studies (11 689 172 participants), of which all had a high risk of bias. Pooled data from 2 studies (n = 3 869 274; very low-quality evidence) showed an inconclusive 42% relative decrease in risk of sudden cardiac death (relative risk = 0.58; 95% CI = 0.23, 1.45), equating to an absolute risk reduction of 0.0016%. The findings were consistent with a potential 77% relative decreased risk to a 45% relative increased risk in participants screened using ECG. Heterogeneity was found to be high, as measured using I2 statistic (71%). Data from the remaining study and abstract were similarly inconclusive.
Conclusions: Existing evidence for the effect of ECG screening is inconclusive and of very low quality. In our meta-analysis, we observed that screening ECG may result in a considerable benefit or harm to participants. Higher-quality studies are needed to reduce this uncertainty.

Objective: To evaluate the quality of the evidence on the incidence of sudden cardiac arrest (SCA) and sudden cardiac death (SCD) in athletes and military members and estimate the annual incidence of SCA and SCD.
Data sources: We searched MEDLINE, Embase, Cochrane CENTRAL, Web of Science, BIOSIS, Scopus, SPORTDiscus, PEDro, and ClinicalTrials.gov from inception to dates between February 21 and July 29, 2019.
Study selection: Studies in which the incidence of SCA, SCD, or both in athletes or military members aged <40 years was reported were eligible for inclusion. We identified 40 studies for inclusion.
Data extraction: Risk of bias (ROB) was assessed using a validated, customized tool for prevalence studies. Twelve had a low ROB, while the remaining 28 had a moderate or high ROB. Data were extracted for narrative review and meta-analysis.
Data synthesis: Random-effects meta-analysis was performed in studies judged to have a low ROB in 2 categories: (1) 5 studies of regional- or national-level data, including athletes at all levels and both sexes, demonstrated 130 SCD events with a total of 11 272 560 athlete-years, showing a cumulative incidence rate of 0.98 (95% CI = 0.62, 1.53) per 100 000 athlete-years and high heterogeneity (I2 = 78%) and (2) 3 studies of competitive athletes aged 14 to 25 years were combined for a total of 183 events and 17 798 758 athlete-years, showing an incidence rate of 1.91 (95% CI = 0.71, 5.14) per 100 000 athlete-years and high heterogeneity (I2 = 97%). The remaining low-ROB studies involved military members and were not synthesized.
Conclusions: The worldwide incidence of SCD is rare. Low-ROB studies indicated the incidence was <2 per 100 000 athlete-years. Overall, the quality of the available evidence was low, but high-quality individual studies inform the question of incidence levels.

The order Cetacea (dolphins, whales, and porpoises) evolved from terrestrial artiodactyls (even‐toed ungulates) around 50 million years ago. This transition from land to water occurred over an evolutionarily short period of less than 10 million years and is considered one of the best examples of macroevolutionary change among vertebrates. Among the sensory systems, the ear has developed a number of unique adaptations to adapt to water borne sounds. We hypothesize that a record of these evolutionary changes is preserved in the prenatal ontogeny. In particular, previous studies on fossils have identified a rotation of the middle ear ossicles with regard to the median plane, as well as a deflection of the axis of the cochlea. It is not clear whether these two movements are related to each other. The purpose of this study is to investigate middle ear development within odontocetes (toothed whales) using two species of dolphin (Stenella attenuata and stenella longirostris). Using Amira and Avizo 3‐D software we investigate the malleus, incus, and stapes over a range of embryological stages and their relationship to inner ear structures, such as the cochlea and semicircular canals. These data are then compared to what is known for adult Stenella. Data is collected on histological sections and CTs of heads of embryologic and fetal specimens. These specimens have either been stained using phosphotungstic acid and microCT scanned (stored in 70% ethanol) or sliced at 7μm and stained using hematoxylin and eosin. We photograph histologic specimens (every other slice) in the region containing the middle and inner ear structures. Avizo and Amira software version 2019.1 are used to reconstruct the middle ear ossicles, cochlea, and semicircular canals of Stenella from micro CT scans and histological specimens. After reconstruction we use the 3‐D images to evaluate changes in the formation and position of the middle ear ossicles through development. The outline of middle ear and cochlear development in these dolphins is similar to that of land mammals. For example, the otic capsule shows the formation of the cochlea, and ossicle formation passes through mesenchymal and cartilaginous stages. The malleus is attached to Meckel’s cartilage and the stapes is associated with the otic capsule. Additionally, there are general similarities in ossicular shape in the later fetal stages, however in earlier embryonic stages the manubrium of the malleus and the crura of the incus are not as apparent as they are in land mammals, possibly foreshadowing the specific adaptations of cetaceans. Changes within the position of the ossicles in relation to one another as well as the cochlea and semicircular canals occur in order for the middle ear to develop into its adult state. In adult odontocetes, the malleus is fused to the tympanic plate through its anterior process, the process gracilius, creating a folded tympanic membrane. These findings support our hypothesis that many of the middle ear adaptations for odontocetes can be visualized in utero. Preliminary evidence suggests that changes in ossicular position replicate cetacean evolution. This research continues to shed light on how and when these unique changes occur to allow cetacean specialization to underwater hearing.

Fournier's gangrene (FG) is a very serious and life threatening emergency. FG is a polymicrobial subset of necrotizing fasciitis affecting the genital region. Most cases have a perianal or colorectal focus; less often the gangrene originates from the urogenital tract or is preceded by trauma or a surgical procedure. FG is a surgical emergency that requires early intervention. Therefore, early recognition by emergency physicians is imperative. The diagnosis of FG during its early stages is often challenging and misdiagnosis is common. If FG is suspected, emergency department management should always include antimicrobial treatment with parenteral broad-spectrum antibiotics, aggressive hemodynamic stabilization and surgical consultation. We report a case of FG in an elderly male with no major risk factors, who presented with right groin and perineal pain that was initially diagnosed as scrotal cellulitis. He was later diagnosed with FG and subsequently developed multi-organ failure, required multiple surgical debridements, and was later transferred to a long-term care facility with poor prognosis. This case is important because it calls attention to the challenges of diagnosing this potentially fatal disease. Emergency physicians must recognize the symptoms of FG because early diagnosis can improve outcomes.

BACKGROUND & AIMS: As many as 45% of patients with gastroesophageal reflux disease (GERD) still have symptoms after receiving once-daily proton pump inhibitor (PPI) therapy. We aimed to compare reflux characteristics and patterns between responders and non-responders to once-daily PPI therapy using combined impedance-pH monitoring. METHODS: Patients who reported heartburn and/or regurgitation at least twice per week for 3 months while receiving standard-dose PPI therapy were assigned to the PPI failure group (n = 16). Patients who reported a complete resolution of symptoms on once-daily PPIs for at least 4 weeks were assigned to the PPI success group (n = 13). We collected demographic data and subjects completed the short-form 36 and the GERD health-related quality of life questionnaires. Patients then underwent upper endoscopy and combined esophageal impedance-pH monitoring while on PPI therapy. RESULTS: Four patients in the PPI success group (31%) and 4 patients in the PPI failure group (25%) had abnormal results from the pH test (P = 1.00). Most of the patients in the PPI failure group (75%) were found to have either functional heartburn or reflux hypersensitivity with GERD. Impedance and pH parameters did not differ significantly between the PPI failure and success group. CONCLUSIONS: We found no difference in reflux characteristics between patients with GERD who had successful vs failed once-daily PPI therapy. Most patients in the PPI failure group (75%) had functional esophageal disorders.
